The sources consulted worldwide <strong>in</strong> mak<strong>in</strong>g this assessment <strong>in</strong>clude literature<br />

written by academics, development agencies, policy analysts, present and<br />

former WHO staff members and health journalists. The official views of some<br />

member states were reviewed selectively <strong>in</strong> literature from <strong>in</strong>dividual countries<br />

and major donor networks. Interviews were conducted with past and present<br />

WHO staff members, consultants and advisers, and other observers. The staff<br />

members <strong>in</strong>cluded people work<strong>in</strong>g at global, regional and country levels <strong>in</strong><br />

a range of specialties and fields, at different levels of seniority and from different<br />

national backgrounds, some newly arrived <strong>in</strong> the organization, others<br />

long-serv<strong>in</strong>g. Many WHO <strong>in</strong>formants felt anxious about speak<strong>in</strong>g openly, and<br />

all were <strong>in</strong>terviewed on the basis that they would not be identifiable. The views<br />

described here represent an aggregate rather than those of any <strong>in</strong>dividual.<br />

Some background<br />

WHO came <strong>in</strong>to formal existence <strong>in</strong> 1948 as the UN specialist agency for<br />

health, <strong>in</strong>corporat<strong>in</strong>g several exist<strong>in</strong>g organizations that represented a long<br />

history of <strong>in</strong>ternational health cooperation. WHO’s objective is the atta<strong>in</strong>ment<br />

by all peoples of the highest possible level of health, def<strong>in</strong>ed as a state<br />

of complete physical, mental and social well-be<strong>in</strong>g and not merely the absence<br />

of disease or <strong>in</strong>firmity. Its constitution also asserts that health is a fundamental<br />

human right and that governments are responsible for the health of their<br />

peoples – bold statements treated warily by governments who equated social<br />

equity and socialized medic<strong>in</strong>e with ‘the Communist threat’ (Lee 1998). Thus<br />

politics and health were <strong>in</strong>separable even at WHO’s birth.<br />

The importance of health to the global political agenda of the day was reflected<br />

<strong>in</strong> the decision to give WHO its own fund<strong>in</strong>g system and a govern<strong>in</strong>g<br />

body of all member states that is still unique among UN specialist agencies.<br />

Its basic composition and overall organizational structure have changed little<br />

s<strong>in</strong>ce 1948. Like other UN non-subsidiary specialist agencies its govern<strong>in</strong>g body<br />

makes its own decisions, but reports annually to the UN. All UN member states<br />

and others may jo<strong>in</strong> it. Through the World <strong>Health</strong> Assembly, its 192 member<br />

states approve the programme of work and budget and decide major policy. A<br />

32–strong executive board with rotat<strong>in</strong>g membership, selected on the basis of<br />

personal expertise rather than country representation (although a geographical<br />

balance is ma<strong>in</strong>ta<strong>in</strong>ed), oversees implementation of assembly decisions (WHO<br />

global website, 2005). Its accountability to its annual global and regional assemblies<br />

of delegations from all member states is unique <strong>in</strong> the UN system, and<br />

offers develop<strong>in</strong>g countries unparalleled opportunities to exert <strong>in</strong>fluence.<br />
