are allow<strong>in</strong>g a runaway agenda to shape new paradigms for the way we see life,<br />

health and health care, with global justice and equity on the los<strong>in</strong>g end. To help<br />

restore the balance, the follow<strong>in</strong>g proposals should be considered:<br />

Democratization of the health research agenda. The national and <strong>in</strong>ternational<br />

systems for sett<strong>in</strong>g the agenda for health research need to be overhauled.<br />

The relative importance of biotechnology research, <strong>in</strong>clud<strong>in</strong>g the study<br />

of the genome, should be weighed aga<strong>in</strong>st research <strong>in</strong>to diseases of poverty<br />

and the social ecology of health and disease. There should be genu<strong>in</strong>e representation<br />

and participation of community groups <strong>in</strong> sett<strong>in</strong>g priorities and<br />

research design.<br />

<strong>Global</strong> equity and justice first. Organizations focus<strong>in</strong>g on health and equity<br />

should <strong>in</strong>sist that genome technologies and their applications are guided by<br />

the core values of national and global equity, human rights <strong>in</strong>clud<strong>in</strong>g gender<br />

rights, and medical ethics (UN 1966 and 2001, WMA 1983, EFMA-WHO 2001,<br />

UNESCO 1997).<br />

<strong>Health</strong> and equity impact and risk assessments. Civil society, and <strong>in</strong>ternational<br />

groups of <strong>in</strong>terested scientists, should demand that states and <strong>in</strong>ternational<br />

organizations like WHO carry out health and equity impact assessments<br />

and risk assessments us<strong>in</strong>g such criteria as <strong>in</strong>ternationally agreed human<br />

rights <strong>in</strong> health and health care. They should be participatory, with genu<strong>in</strong>e<br />

representation of civil society, and free from pressures aris<strong>in</strong>g from <strong>in</strong>ternational<br />

economic and donor policies. Assessments should <strong>in</strong>clude the potential<br />

effects of different scenarios of genome applications on health and equity,<br />

nationally and <strong>in</strong>ternationally, <strong>in</strong> different social and health care systems.<br />

Expertise and experience on risk assessments and the precautionary pr<strong>in</strong>ciple<br />

can be drawn from environmental campaigns.<br />

Equitable access to and use of knowledge. States and research funders<br />

should develop ways by which researchers give up or selectively forgo patent<br />

rights to help make useful <strong>in</strong>ventions cheaply available for all.<br />

Overhaul of regimes for <strong>in</strong>tellectual property rights. This is needed to create<br />

a last<strong>in</strong>g solution to the crisis <strong>in</strong> the knowledge economy (see also part B,<br />

chapter 2). Solutions <strong>in</strong>clude reduc<strong>in</strong>g the length and coverage of patents, and<br />

liberal provisions for governments and UN <strong>in</strong>stitutions to buy patent rights<br />

from patent-holders if this is <strong>in</strong> the public <strong>in</strong>terest, and/or arrangements for<br />

compulsory licens<strong>in</strong>g (Love 2003, Baker 2004). Pressure on countries to accept<br />

deals unfavourable to their populations should be ended. There should be<br />

credible monitor<strong>in</strong>g systems and sanctions. These improvements should be<br />

seen <strong>in</strong> the context of the need to establish a fair and equitable <strong>in</strong>ternational<br />

trade system.<br />
