that research may be commercialized, potentially block<strong>in</strong>g its use for poor,<br />

un<strong>in</strong>sured people and develop<strong>in</strong>g countries. Patients should be offered an<br />

<strong>in</strong>formed choice <strong>in</strong>clud<strong>in</strong>g a warn<strong>in</strong>g of that possibility.<br />

The current trend for doctors and nurses to <strong>in</strong>itiate the collection of huge<br />

amounts of biomedical data about their patients is both scientifically and ethically<br />

wrong. Truly <strong>in</strong>formed consent (Thornton 2003) and patient autonomy<br />

are often ignored and the medical benefits overstated (Getz et al. 2003). The<br />

negative effects on patients <strong>in</strong>clude anxiety about be<strong>in</strong>g at risk (Melzer and<br />

Zimmern 2002) and becom<strong>in</strong>g <strong>one</strong> of the worried well. Issues <strong>in</strong>clude the<br />

conflict between act<strong>in</strong>g as a truly confidential counsellor on potential genetic<br />

conditions and a collector of data for the purposes of the adm<strong>in</strong>istration and<br />

control of health care, public health and risk selection.<br />

Doctors may <strong>in</strong>creas<strong>in</strong>gly be asked to play a role <strong>in</strong> requests for selection of<br />

sex or other features of babies. Such requests may be used as conditions for<br />

marriage and lead to marg<strong>in</strong>alization or exclusion and the further control and<br />

oppression of women and their reproductive rights. The eugenicist flavour of<br />

some of the proposed applications of genetic research, and their implications<br />

for people with disabilities, is a l<strong>in</strong>ked concern discussed <strong>in</strong> more detail <strong>in</strong> part<br />

C, chapter 2 on disability.<br />

Trust <strong>in</strong> health professionals will not be greatly underm<strong>in</strong>ed by these<br />

developments <strong>in</strong> the short or medium term. Doctors and nurses come top<br />

<strong>in</strong> many countries’ surveys of who is most trusted and respected, and this is<br />

unlikely to change. However, the commercialization of health care and commercial<br />

risk selection are progress<strong>in</strong>g fast. Aga<strong>in</strong>st this background, can health<br />

care, and can doctors and nurses, be trusted? Are they Trojan horses, whether<br />

they like it or not? How can they help ensure that patients’ <strong>in</strong>terests come<br />

first, <strong>in</strong>dividually and globally? <strong>Health</strong> professionals, their organizations and<br />

health-related NGOs need to respond to these questions.<br />

Conclusions and proposals<br />

The assumptions and activities of the scientific and commercial enterprises<br />

around biotechnology, especially genome technology, merit close scrut<strong>in</strong>y. A<br />

legal armamentarium has emerged to br<strong>in</strong>g the human body and other life<br />

forms with<strong>in</strong> the ambit of <strong>in</strong>tellectual property, and present life as a commodity<br />

which can be patented, traded and made to yield a profit. The quest for competitive<br />

advantage and dom<strong>in</strong>ance <strong>in</strong> biotechnology has spurred governments and<br />

corporations to promote the privatization and commercialization of biotechnological<br />

knowledge. Current developments also threaten reproductive rights and<br />

underm<strong>in</strong>e global and national equity. Governments, <strong>in</strong>dustry and scientists<br />
