‘As fem<strong>in</strong>ists and women’s rights activists we must cont<strong>in</strong>ue to ma<strong>in</strong>ta<strong>in</strong> a<br />

critical surveillance of the implementation of MDGs <strong>in</strong> order to protect our<br />

<strong>in</strong>terests <strong>in</strong> equality and nondiscrim<strong>in</strong>ation,’ warns Sunila Abeyeskera from<br />

Sri Lanka (ARROW 2004).<br />

The goal to reduce maternal mortality is nevertheless important as more<br />

women are dy<strong>in</strong>g <strong>in</strong> childbirth and suffer<strong>in</strong>g from chronic ill health follow<strong>in</strong>g<br />

complicated deliveries than 10 years ago. Maternal mortality rema<strong>in</strong>s a priority<br />

<strong>in</strong> Sub-Saharan Africa and south Asia <strong>in</strong> particular. However, the absence<br />

of other reproductive health and rights concerns reflects the UN’s reluctance<br />

to recognize or take a strong stand on other sexual and reproductive health<br />

and rights issues <strong>in</strong> many arenas, particularly women’s autonomy to choose,<br />

abortion and sexuality (DAWN 2004).<br />

The universal rise of fundamentalism has fired a ‘morality’ debate with<br />

many examples of how these conservative approaches are underm<strong>in</strong><strong>in</strong>g the<br />

sexual and reproductive health and rights agenda. One is the rule imposed by<br />

President Bush <strong>in</strong> 2001 prohibit<strong>in</strong>g overseas NGOs from receiv<strong>in</strong>g US government<br />

aid if they promote or provide referrals for abortion. In 2002, the US<br />

government withheld US$ 34 million promised to UNFPA because it said it<br />

funded abortion <strong>in</strong> Ch<strong>in</strong>a. Meanwhile it is fund<strong>in</strong>g the anti-abortion lobby<br />

and promotion of abst<strong>in</strong>ence overseas (us<strong>in</strong>g the ABC approach: Abst<strong>in</strong>ence<br />

until marriage, Be faithful and Condoms where appropriate (Jacobson 2003)).<br />

The Vatican and conservative states consistently attack the sexual and reproductive<br />

health and rights agenda. Such strategies threaten many sexual and<br />

reproductive health and rights projects <strong>in</strong> the global South and are lead<strong>in</strong>g to<br />

<strong>in</strong>creased unsafe abortion, closure of family plann<strong>in</strong>g cl<strong>in</strong>ics and shortages of<br />

contraceptive supplies.<br />

Lack of donor fund<strong>in</strong>g is not the only obstacle. Broader underly<strong>in</strong>g economic<br />

and political conditions are underm<strong>in</strong><strong>in</strong>g women’s health and their<br />

control over safe sex and childbear<strong>in</strong>g. <strong>Health</strong> services are <strong>in</strong> decl<strong>in</strong>e and<br />

economically poor women <strong>in</strong> particular have little or no access to reproductive<br />

or other health services. Recent reports on poverty have tracked the deterioration<br />

of women’s health, particularly those who are economically vulnerable.<br />

<strong>Global</strong> <strong>in</strong>equalities <strong>in</strong> <strong>in</strong>come and health have been grow<strong>in</strong>g and hunger has<br />

<strong>in</strong>creased despite fall<strong>in</strong>g population growth, as detailed <strong>in</strong> part D, chapter 3<br />

(Yong et al. 2000, Desai 2004, Rao 2004).<br />

Many activists are develop<strong>in</strong>g an alternative vision far more critical of the<br />

development establishment and globalization that goes beyond the Cairo consensus.<br />

They believe Cairo went along too readily with the neoliberal agenda<br />

that is prov<strong>in</strong>g so disastrous for the poor, the vast majority of whom are rural<br />
