migration from rural to urban and public to private, <strong>in</strong>creas<strong>in</strong>g <strong>in</strong>equity along<br />

the way (see Figure B3.4). There are a number of drivers. There is a grow<strong>in</strong>g<br />

demand for health care from the age<strong>in</strong>g populations of the developed world. At<br />

the same time there is <strong>in</strong>adequate local production of health workers <strong>in</strong> some<br />

of these countries – the US, for example, will need over a million more nurses<br />

by 2010 (US Bureau of Labor Statistics 2004). Active recruitment – proactive<br />

hir<strong>in</strong>g and advertis<strong>in</strong>g <strong>in</strong> low-<strong>in</strong>come countries by recruitment agencies act<strong>in</strong>g<br />

on behalf of rich country health systems – has also encouraged migration.<br />

These demand pressures have been re<strong>in</strong>forced by globalization and commercialization.<br />

Research <strong>in</strong> Ghana shows that technological change, notably<br />

the Internet, has dramatically <strong>in</strong>creased knowledge of jobs and conditions<br />

elsewhere, and developed world health journals with recruitment advertis<strong>in</strong>g<br />

are widely available. Commercial <strong>in</strong>vestment <strong>in</strong> recruitment agencies is also<br />

facilitat<strong>in</strong>g migration, mak<strong>in</strong>g obta<strong>in</strong><strong>in</strong>g visas, jobs and accommodation much<br />

easier (Mensah et al. 2005).<br />

The <strong>in</strong>ternational bra<strong>in</strong> dra<strong>in</strong> is also driven by a variety of other factors<br />

such as the widespread collapse of health systems <strong>in</strong> many low-<strong>in</strong>come coun-<br />
