One-time Conversion Bonus (all LHM’s). Any employee enrolled in a Buy-Up Plan (PPO, EPO and/or POS at SFMC) at the time of ratification of this contract who elects to change from a Buy-Up Plan (PPO, EPO and/or POS at SFMC) to a HMO (POS at SLRH) during Open Enrollment in November 2009 and remains enrolled in the HMO in 2010 shall receive a bonus in the first full pay period of January, 2010. If the employee remains enrolled in the HMO (POS at SLRH) plan for calendar year 2011, the employee will receive an additional bonus in January 2011 according to the following schedule: 91

C. One-time Conversion Bonus (all LHM’s). Any employee enrolled in a Buy-Up<br />

Plan (PPO, EPO and/or POS at SFMC) at the time <strong>of</strong> ratification <strong>of</strong> this contract<br />

who elects to change from a Buy-Up Plan (PPO, EPO and/or POS at SFMC) to a<br />

HMO (POS at SLRH) during Open Enrollment in November 2009 and remains<br />

enrolled in the HMO in 2010 shall receive a bonus in the first full pay period <strong>of</strong><br />

January, 2010. If the employee remains enrolled in the HMO (POS at SLRH)<br />

plan for calendar year 2011, the employee will receive an additional bonus in<br />

January 2011 according to the following schedule:<br />
