To be eligible to receive a certification bonus, an employee must first timely submit evidence of the initial receipt of the certification to the Employer. Thereafter, the employee will be eligible for payment of the maintenance bonus on the anniversary date of the initial bonus. To obtain payment the employee must submit their request for payment within one month of the date of the anniversary of the initial bonus. Employees who already have the credential/ certification will be eligible for payment of the maintenance bonus on the anniversary date of their credential/ bonus. All bonuses are subject to normal and customary withholdings, as required by law. Credentials/ Certifications include: Certification Gerontology CPHQ CCM – Case Management RDCS RDMS National EKG National CT National MRI National Mammography National CV (Cardiovascular) National CI (Cardiac Invasive) National VS (Vascular Sonography) National BS (Breast Sonography) RCIS – Registry of Cardiovascular Invasive Society National Surgical Tech Certification VI – Vascular Interventional Radiographer For any additional credentials/ certifications not listed above, employees are free to submit for payment for the same but the Employer and the Union must agree upon the credential/ certification before any payment is made. ARTICLE 18: WORK WEEK A. Definition of Overtime: At Seton, Saint Louise and O’Connor Only: The employee’s workweek shall be designated by the Employers and shall be a consecutive period of seven (7) days. Straight-time hourly rates shall apply up to a maximum of eighty (80) hours per pay period, forty (40) hours per week for employees not working on an “8 and 80” basis, or eight (8) hours per day. All work in excess of eight (8) hours per day, forty (40) hours per week or eighty (80) hours per pay period 54

shall be paid at the rate of one and one half times (1½ X) the basic straight-time hourly rate. All work in excess of twelve (12) hours within a twenty-four (24) hour period shall be paid at the rate of two times (2 X) the basic straight-time hourly rate. All work on the seventh (7th) consecutive day of the work week shall be paid for at one and one-half times (1 ½ X) the basic straight-time hourly rate, two times (2X) at Seton. Each employee shall receive two (2) consecutive days off each pay period, provided that the days off may be split or rotated at the employee’s request without penalty. When an employee works two (2) full consecutive shifts, totaling fifteen and one half (15 ½) hours, the second shift will be compensated at double the straight time wage rate. No employee shall be required to work two (2) shifts within a period of twenty-four (24) hours except in cases of emergency. The definition of workweek or workday is not intended to avoid overtime payments for consecutive hours/days worked beyond the defined period. The provisions of this Section shall not apply to employees working on an alternative workweek schedule as provided in Section C, Alternative Workweeks, below. At Seton Only: As an exception to the provisions of the preceding paragraph, where the employees in a department have elected to adopt the weekend off provisions of Section G of this Article, and have not waived the provisions of said Section G, it is not required that employee in such department be paid one and one-half times (1½X) the basic straight-time hourly rate for work in excess of forty (40) hours per week or two times (2X) the basic straight time rate for work on the seventh (7th) consecutive day of the work week, or that employees in such department shall receive two (2) consecutive days off each week without split or rotation. Where employees in a department have not waived the weekend off provisions of Section G of this Article, all employees in such department shall be paid weekly overtime in accordance with the eighty (80) hour option under the Federal Fair Labor Standards Act, may be required to work back-to-back workweeks, and may have their days off rotated or split without penalty. However, the provisions of Section C of this Article shall apply to such employees. All work on the seventh (7th) consecutive day of the work week shall be paid for at two times (2 X) the basic straight-time hourly rate. B. Days Off. At Seton, Saint Louise and O’Connor Only: An employee who works more than seven (7) consecutive days without a day off shall be compensated thereafter at time and one-half (1 ½ ) the employee’s basic straight-time hourly rate for each day worked or portion thereof until granted a day off.
