On the Approval of the Veterinary Medicine Registration Certificate Forms
Pursuant to the sub-paragraph b of 4 th paragraph of an Article 38 of the Georgian law on Medicine and
Pharmaceutical Activity and Article 8 th of the Georgian Law On Forms of Strict Accounting,
I order:
1. To be approved by the attached order:
a) Veterinary Medicine Registration Certificate Form (Annex 1);
b) Accounting Journal of the Veterinary Medicine Registration Certificate (Annex 2);
c) Registering and Accounting Rules of the Veterinary Medicine Registration Certificate (Annex 3);
d) Form for Writing off the Useless Veterinary Medicine Registration Certificates (Annex 4).
2. Order to be enforced upon the promulgation.

a. For the inaccuracy during the filling;
b. For the damage of the Certificates;
c. For the Polygraphic Defects.
2. Writing off of the invalid Certificates, is done by the elaboration of the Writing Off Documents,
indicating the exclusive number and the reason.
3. The exclusive number of the invalid Certificates is entered in an Accounting Journal.
4. The utilization of the invalid Certificates is done by the established rules of the Georgian
Legistlation.
Article 5
Storage, Accounting and Issuance Control
1. Storage, Accounting and Issuance is controlled by the NFA.
2. The NFA takes inventory of the Strict Certificates in the end of every Calendar Year, according
to the Legislation.
