8. Cl<strong>in</strong>ical c<strong>are</strong> issues: preconception<br />
Learn<strong>in</strong>g po<strong>in</strong>ts<br />
• In <strong>the</strong> CEMACH survey of maternity services, only 17% of maternity units <strong>in</strong> England, Wales and<br />
Nor<strong>the</strong>rn Ireland reported provid<strong>in</strong>g structured multidiscipl<strong>in</strong>ary preconception c<strong>are</strong> for women<br />
with diabetes. A quarter of women <strong>in</strong> <strong>the</strong> enquiry <strong>we</strong>re reported to have had preconception c<strong>are</strong><br />
at a multidiscipl<strong>in</strong>ary hospital cl<strong>in</strong>ic.<br />
• Two thirds of women <strong>in</strong> <strong>the</strong> enquiry had suboptimal preconception c<strong>are</strong>. One of <strong>the</strong> ma<strong>in</strong><br />
underly<strong>in</strong>g issues was failure of health professionals to provide appropriate c<strong>are</strong> such as<br />
preconception advice and higher dose folic acid. There was poor documentation of<br />
pre-<strong>pregnancy</strong> counsell<strong>in</strong>g.<br />
• Suboptimal preconception c<strong>are</strong> was associated with poor <strong>pregnancy</strong> outcome.<br />
8.1 Introduction<br />
Women with type 1 and type 2 diabetes represent a high-risk population dur<strong>in</strong>g <strong>pregnancy</strong>. Cl<strong>in</strong>icians have<br />
a responsibility to provide appropriate <strong>in</strong>formation, effective fetal and maternal surveillance, and timely<br />
health c<strong>are</strong> <strong>in</strong>terventions to achieve <strong>the</strong> <strong>best</strong> possible outcome for <strong>the</strong> woman and her baby. The <strong>Diabetes</strong><br />
National Service Framework (NSF) emphasises <strong>the</strong> importance of an effective multidiscipl<strong>in</strong>ary team, both<br />
to support and empo<strong>we</strong>r women to plan <strong>the</strong>ir pregnancies, optimise glycaemic control before conception<br />
and improve <strong>the</strong> quality of antenatal c<strong>are</strong> dur<strong>in</strong>g <strong>pregnancy</strong>. 1 Ho<strong>we</strong>ver, <strong>the</strong> recent CEMACH descriptive<br />
study of 3808 pregnancies to women with type 1 and type 2 diabetes found that <strong>the</strong> majority of women<br />
with pre-exist<strong>in</strong>g diabetes <strong>in</strong> England, Wales and Nor<strong>the</strong>rn Ireland <strong>are</strong> poorly prep<strong>are</strong>d for <strong>pregnancy</strong>. 2<br />
8.2 Preconception c<strong>are</strong> services<br />
The <strong>in</strong>creased risk of fetal congenital anomalies (with <strong>the</strong> ma<strong>in</strong> contributors be<strong>in</strong>g neural tube and cardiac<br />
defects) 3 and per<strong>in</strong>atal death <strong>in</strong> babies of women with pre-exist<strong>in</strong>g diabetes 2 can be m<strong>in</strong>imised by ensur<strong>in</strong>g<br />
good glycaemic control before and dur<strong>in</strong>g <strong>pregnancy</strong> 4 and by commenc<strong>in</strong>g high dose (5mg daily) folic acid<br />
before conception. 5<br />
Dur<strong>in</strong>g <strong>the</strong> CEMACH survey of maternity services (2004) 6 , only 17% of maternity units <strong>in</strong> England, Wales<br />
and Nor<strong>the</strong>rn Ireland reported that <strong>the</strong>y provided a multidiscipl<strong>in</strong>ary diabetes preconception service. The<br />
CEMACH descriptive study found that only 35% of women <strong>we</strong>re documented <strong>in</strong> <strong>the</strong> medical notes to have<br />
had pre-<strong>pregnancy</strong> counsell<strong>in</strong>g, with only a tenth of <strong>the</strong>se seen <strong>in</strong> a formal preconception cl<strong>in</strong>ic. 2 Dur<strong>in</strong>g <strong>the</strong><br />
enquiry, general practitioners and adult diabetes services <strong>we</strong>re also asked where preconception c<strong>are</strong> had<br />
been provided.<br />
