Before Surgery Check List
What to do 4-6 weeks prior to surgery
Contact Your Insurance Company
Before surgery, you will need to call your insurance company. you will need to ask them if
pre-authorization, pre-certification, second opinion or a referral form is needed before your
surgery. It is important to make this call if things are missed they may not pay for some
portions or delay your surgery. Complete the “Center for Advanced Joint Replacement
Insurance Checklist” form found on the next page and have this information ready when
you talk to your case manager.
If you do not have insurance, please tell the registration staff that you will need help in
making payment arrangements.
Pre-Register
After your surgery has been scheduled, the admitting department will call you to gather
your “pre-registration information” by phone. You will need to have the following
information ready:

Your full legal name and address, including county
Home phone number
Religion
Marital status
Social Security Number
Name of insurance holder, his or her address and phone number
Name of insurance company, mailing address, policy and group number
Your employer, address, phone number and occupation
Your retirement date, if applicable
Name, address and phone number of someone to notify in case of emergency (this can be
the same as the nearest relative)
Get Medical And Anesthesia Clearance
When you are scheduled for surgery, your surgeon will tell you whether you need to see
your primary care doctor and/or a specialist before your surgery. Your case manager will
call you to get medical information for the anesthesiologist 1-2 weeks before your operation.
You will need to have a list of all your medications and the doses. The case manager may
ask you to see your primary care doctor and/or a specialist after going over your history
