Q 8. Ans. What is the procedure of changing address for the students enrolled in Study Material Package or All India Test Series? Write an application to the "Course Coordinator (Distance Learning Division) mentioning complete details of course in which you enrolled viz. Name, Old Address, Contact Numbers, Course Name, Roll No. and the new address where you want to receive the Study Material / Test papers along with contact number with photocopy of Acknowledgement/ Identity Card of Career Point Kota. Q9. What is the procedure of changing a Test Centre for All India Test Series (At Centre)? Ans. Write an application to the "Course Coordinator, Distance Learning Division, Career Point" mentioning reason for changing the Test Centre, Course Name, Roll No., old Test Centre and the name of new Test Centre along with photocopy of Acknowledgement / Identity Card of Career Point Kota along with a Test Centre change fee of ` 300/- (` Three Hundred Rupees) in the form of DD in the favor of "Career Point" payable at Kota) Q10. HOW TO FILL OMR APPLICATION FORM For Distance Learning Courses 1. OMR Application Form is a computerized form. It will be processed by OMR scanning machine. Therefore, you are advised to fill it very carefully as per the instructions given below, otherwise your form will be rejected. 2. Write only English Block Letters and Numerals in the Rectangular Boxes and darken the corresponding oval completely. 3. Use only HB Pencil to darken the ovals and Blue/Black ball pen for all other entries. 4. Do no Staple, Pin, Fold, Wrinkle, Scrabble, Tear, and Wet or Damage the form in any way. Damaged forms are liable to be rejected. 5. Completely filled sample Application Form is given for your reference in the pages to follow. COLUMN NO.
