This analysis would fit with the tentative suggestion that heroong>inong> use is still diffusong>inong>g ong>inong> ong>Pakistanong> to new areas, ong>inong>cludong>inong>g more rural areas, whilst at the same time stabilizong>inong>g, or even possibly slightly declong>inong>ong>inong>g, ong>inong> some of the urban centres where use has been longest established. This picture remaong>inong>s prelimong>inong>ary and requires further ong>inong>vestigation before any firm conclusion should be drawn. However, the “last-year” overall trend measure ong>inong>dicates a small declong>inong>e ong>inong> heroong>inong> use ong>inong> the provong>inong>ce of Punjab (–0.9) (see table 9). 4 2 0 Hashish Heroong>inong> Other opiates This compares to ratong>inong>gs of 10 and 18 for Song>inong>dh and Balochistan, respectively, and 1.7 ong>inong> NWFP. This poong>inong>t is illustrated ong>inong> figure II. Overall, the data suggest that ong>inong> Punjab heroong>inong> prevalence may have been stable or even ong>inong> possible declong>inong>e over the last five years. Elsewhere, it would ong>inong>dicate no great change ong>inong> NWFP, where reported heroong>inong> use is low ong>inong> any respect, and ong>inong>dicate ong>inong>creases ong>inong> Song>inong>dh and ong>Drugong> ong>inong>jection Solvents Punjab Song>inong>dh Balochistan NWFP Note: high scores ong>inong>dicate ong>inong>creased reports that drug ong>abuseong> has ong>inong>creased ong>inong> the last five years. Alcohol If a mixed picture is found for heroong>inong> use this is not the case for drug ong>inong>jection where data give cause for considerable concern. Increased ong>inong>jectong>inong>g is reported ong>inong> all provong>inong>ces although the ong>inong>crease ong>inong> NWFP is margong>inong>al. However Song>inong>dh, Balochistan, and Punjab all report strong upward trends. Injectong>inong>g is reported to be ong>inong>creasong>inong>g more strongly ong>inong> urban rather than rural locales, but for both the trend is upwards. Key ong>inong>formants’ perception of long term trends ong>inong> hashish and charas ong>abuseong> appear uniformly up with some the highest overall ratong>inong>gs ong>inong> Balochistan and NWFP. Opium ong>abuseong> on the other hand was reported to be stable or ong>inong> declong>inong>e ong>inong> all provong>inong>ces with the exception of Balochistan. Trends ong>inong> alcohol use were reported to be up, as was the ong>abuseong> of solvents (glue, etc), with the locales ong>inong> the provong>inong>ce of Punjab reportong>inong>g the greatest overall ong>inong>crease. 18

High overall scores were also found for upward trends ong>inong> the use of psychotropic drugs. Given the earlier fong>inong>dong>inong>gs that these drugs are reported as the most commonly used drug type by women this may imply that drug ong>abuseong> among women ong>inong> ong>Pakistanong> is ong>inong>creasong>inong>g. Data ong>inong> this report do not explore the use of psychoactive substance by women ong>inong> any great detail. However, data do suggest that this is a topic that merits further attention. Perceptions of trends ong>inong> drug ong>abuseong> over the last year were also explored. This is a topic ong>inong>cluded ong>inong> the UNDCP annual reports questionnaire (ARQ). ong>Drugong> ong>inong>formation systems often report on yearly trends so these data may have utility for any future on-goong>inong>g drug ong>abuseong> monitorong>inong>g activity ong>inong> ong>Pakistanong>. Data from the whole sample can be found ong>inong> table 8, urban versus rural comparisons ong>inong> annex table A.II.5 and provong>inong>cial breakdowns ong>inong> annex table A.II.6. Once more, an overall ratong>inong>g has been calculated to summarize these data and this can be found ong>inong> annex table A.II.9. The overall picture of perceived short-term trends broadly corresponds to the data for trends over the longer time period. As noted above, trends ong>inong> heroong>inong> ong>abuseong>, with the exception of Balochistan, appear flat or even down slightly (Punjab). ong>Drugong> ong>inong>jection is regarded as ong>inong>creasong>inong>g, as is hashish and charas ong>abuseong> and agaong>inong> psychotropic drugs receive a particularly high score on the overall ratong>inong>g for a risong>inong>g short-term trend. 19
