Introduction<br />

These guidelines on paediatric life support are based on two<br />

main principles: (1) the incidence of critical illness, particularly<br />

cardiopulmonary arrest, and injury in children is much lower than<br />

in adults; (2) most paediatric emergencies are served primarily by<br />

providers who are not paediatric specialists and who have limited<br />

paediatric emergency medical experience. There<strong>for</strong>e, guidelines on<br />

paediatric life support must incorporate the best available scientific<br />

evidence but must also be simple and feasible. Finally, international<br />

guidelines need to acknowledge the variation in national and<br />

local emergency medical infrastructures and allow flexibility when<br />

necessary.<br />

The process<br />

The <strong>European</strong> <strong>Resuscitation</strong> <strong>Council</strong> (ERC) published guidelines<br />

<strong>for</strong> paediatric life support (PLS) in 1994, 1998, 2000<br />

and 2005. 1–5 The latter two were based on the International<br />

Consensus on Science published by the International Liaison Committee<br />

on <strong>Resuscitation</strong> (ILCOR). 6–8 This process was repeated in<br />

2009/<strong>2010</strong>, and the resulting Consensus on Science with Treatment<br />

Recommendations (CoSTR) was published simultaneously in<br />

<strong>Resuscitation</strong>, Circulation and Pediatrics. 9,10 The PLS Working Party<br />

of the ERC has developed the ERC PLS <strong>Guidelines</strong> based on the<br />

<strong>2010</strong> CoSTR and supporting scientific literature. The guidelines <strong>for</strong><br />

resuscitation of babies at birth are now covered in Section 7. 11<br />

Summary of changes since the 2005 <strong>Guidelines</strong><br />

Guideline changes have been made in response to convincing<br />

new scientific evidence and to simplify teaching and retention.<br />

As be<strong>for</strong>e, there remains a paucity of good-quality evidence on<br />

paediatric resuscitation. There<strong>for</strong>e to facilitate and support dissemination<br />

and implementation of the PLS <strong>Guidelines</strong>, changes have<br />

been made only if there is new, high-level scientific evidence or<br />

to ensure consistency with the adult guidelines. The feasibility of<br />

applying the same guidance <strong>for</strong> all adults and children remains<br />

a major topic of study. Major changes in these new guidelines<br />

include:<br />

Recognition of cardiac arrest<br />

Healthcare providers cannot reliably determine the presence<br />

or absence of a pulse in less than 10 s in infants or children. 12,13<br />

There<strong>for</strong>e pulse palpation cannot be the sole determinant of cardiac<br />

arrest and the need <strong>for</strong> chest compressions. If the victim is<br />

unresponsive, not breathing normally, and there are no signs of life,<br />

lay rescuers should begin CPR. Healthcare providers should look <strong>for</strong><br />

signs of life and if they are confident in the technique, they may add<br />

pulse palpation <strong>for</strong> diagnosing cardiac arrest and decide whether<br />

they should begin chest compressions or not. The decision to begin<br />

CPR must be taken in less than 10 s. According to the child’s age,<br />

carotid (children), brachial (infants) or femoral pulse (children and<br />

infants) checks may be used. 14,15<br />

Compression ventilation ratios<br />

The compression ventilation (CV) ratio used <strong>for</strong> children should<br />

be based on whether one, or more than one rescuer is present. 16 Lay<br />

rescuers, who usually learn only single-rescuer techniques, should<br />
