Who is eligible: Primary Care Physicians, Nurse Practitioners, Certified Nurse-Midwives, Physician Assistants, Dentists and Dental Hygienists, Mental Health Professionals, including Clinical Psychologists, Clinical Social Workers, Marriage and Family Therapists, and Psychiatric Nurse Specialists. For further information or application materials, contact your State Health Resources & Services Administrator. • NSDAR - National Society Daughters of the American Revolution www.dar.org NSDAR awards the following scholarships through the DAR Scholarship Committee. All applicants must be citizens of the US and must attend, or plan to attend, an accredited college or university in the US. Occupational Therapy Scholarship – a one time $500 scholarship for students who are in financial need and have been accepted or are attending an accredited school of Occupational Therapy including art, music or physical therapy . Deadline February 15 th or August 15 th . Lillian and Arthur Dunn Scholarship – a $2,000 scholarship awarded for up to four years to well qualified, deserving sons and daughters of members of the NSDAR for four years of college. DiPietro Memorial Scholarship http://www.pwd.org/about/Scholarship.php Criteria: o Students must demonstrate financial need. o Students must live in PWD’s service territory (Portland,South Portland, Westbrook, Windham, Gorham, Raymond, Cumberland, Falmouth, Scarborough, Cape Elizabeth). o Students must have a declared major that is related to PWD’s mission to protect public health and the environment (e.g., environmental studies, sciences, engineering, medical, dental, trades, public administration). o Preference will be given to a student that attends or plans to attend an accredited college or university (Husson College, University of Southern Maine, University of New England, Southern Maine Community College) in PWD’s member communities. o Scholarship will be given on a one-time basis; not "renewable." o Preference will be given to students who have an affiliation with the Portland Water District. o Preference will be given to students that demonstrate educational excellence through GPA, SAT, academic honors, etc. o Preference will be given to students that demonstrate a commitment to the community through involvement. Deadlines: The $1,500 scholarship will be awarded annually to a deserving student. The scholarship committee, a sub committee of the Board of Trustees, will review applications in the summer and award the scholarship in September. Who is eligible to apply? The Rumford Hospital and Auxiliary Health Professional Student Loan Program is open to students living in the area served by RH who have been accepted into programs of study in the following fields: Nursing; Radiology, Laboratory and Physical Therapy. Scholarship amount: Up to $8,000. How are the scholarships awarded? Awards are based on academics as well as the future needs of the Rumford Hospital, upon proof of satisfactory completion of the first semester for each year. This student loan is automatically renewed with proof of successful completion of the first semester. Completed application must be received by May 1 of the award year. • Safety & Health Council of Northern New England – Past Presidents Scholarship The Safety & Health Council will award a $500. or greater scholarship(s) annually to New Hampshire or Maine resident(s)* majoring in a safety or health program. The scholarship(s) will be awarded upon recommendation of the Past Presidents Scholarship Committee. The scholarship(s) will be open to students who have completed their sophomore year and the award will be based upon scholastic achievement and professional potential. Eligibility: Be a resident of the States of New Hampshire or Maine* Be a currently enrolled degree candidate with a safety, health or environmental studies related major. Possess a cumulative grade point average of at least 2.5, and 3.0 in a safety or health related major. Filing of Application materials: Completed application by April 1, 2005 Official transcript from College Minimum of three letters of recommendation from college instructors, administrators, staff support services, and/or other professional references. The selection of the recipient(s) will be made by the Past Presidents Scholarship Committee. Recipient(s) and funds will be presented to the recipient(s) at the Council Office in Westbrook, Maine or Concord, New Hampshire. Non-recipients will also be notified. *Residence or residency shall mean a person’s principal place of abode or domicile. This year, SEMPA will award one-$1,000 scholarship and four-$500 scholarships. Applicants must be current members in good standing and be enrolled in an approved Physician Assistant Program.
