Programs: Physical <strong>The</strong>rapy, Occupational <strong>The</strong>rapy<br />

Requirements: <strong>The</strong> program is open <strong>to</strong> students who are U.S. citizens at junior level in college or above.<br />

Students must be enrolled in an accredited program by <strong>the</strong> appropriate health <strong>the</strong>rapy pr<strong>of</strong>ession authority in<br />

physical <strong>the</strong>rapy, occupational <strong>the</strong>rapy, speech-language pathology or audiology and must be able <strong>to</strong><br />

demonstrate financial need. Approximately $150,000 is awarded annually.<br />

• American Academy <strong>of</strong> Physican Assistants Foundation Scholarship<br />

Application ]<br />

<strong>The</strong> Physician Assistant Foundation has awarded more than 750 <strong>scholarships</strong> <strong>to</strong>taling more than<br />

$1,000,000 <strong>to</strong> deserving PA students. Any student member <strong>of</strong> <strong>the</strong> American Academy <strong>of</strong> Physician<br />

Assistants who is attending an ARC-PA-accredited PA program and is in <strong>the</strong> pr<strong>of</strong>essional phase <strong>of</strong> his or<br />

her program is eligible <strong>to</strong> apply. Scholarships are awarded based on <strong>the</strong> availability <strong>of</strong> funds provided<br />

through contributions from AAPA members, individuals, and corporations. Previous PA Foundation<br />

scholarship recipients are NOT eligible <strong>to</strong> apply.<br />

This application form provides <strong>the</strong> PA Foundation Scholarship Committee with important information.<br />

Please read each question carefully and answer each question fully. It is imperative that you meet <strong>the</strong><br />

established February 1 deadline. INCOMPLETE APPLICATIONS ARE INELIGIBLE FOR REVIEW.<br />

Applications will be reviewed and judged on <strong>the</strong> basis <strong>of</strong> financial need, academic record, community<br />

and pr<strong>of</strong>essional activities, and future goals as a physician assistant. You will be notified in May 2006 <strong>of</strong><br />

<strong>the</strong> Scholarship Committee's decision. All monies awarded will be distributed in May 2006, and all<br />

recipients will be recognized in published format at <strong>the</strong> AAPA's 34th Annual PA Conference in San<br />

Francisco, CA.<br />

• American Lung Association Golf Scholarship – Medical/Health Related Studies<br />

American Lunch Association <strong>of</strong> Maine <strong>of</strong>fers three $500 <strong>scholarships</strong> <strong>to</strong> eligible students involved in <strong>the</strong> game<br />

<strong>of</strong> golf (players or employees <strong>of</strong> a golf facility), and who are pursuing fur<strong>the</strong>r education in a medical/health<br />

related field. <strong>The</strong>se <strong>scholarships</strong> <strong>have</strong> <strong>been</strong> established by ALA-ME <strong>to</strong> recognize <strong>the</strong> golf courses who support<br />

<strong>the</strong> Golf Privilege Card.<br />

Criteria: Scholarships will be awarded based on academic achievement, financial need, references, review <strong>of</strong><br />

completed application and <strong>the</strong> applicant must be a non-<strong>to</strong>bacco user. Three $500 <strong>scholarships</strong> for graduating<br />

Maine High School Seniors.<br />
