Award is open to any student officially recognized as an undergraduate health education major at any four year
University/college; must currently be enrolled full time (12 hrs) for both the Fall and Spring semesters in which
the recipient is applying; must have sophomore, junior, or senior at the time of application; must have a
minimum, current overall GPA of 3.25 on a 4.0 scale. AAHE application deadline, December 1st.

Programs: Physical Therapy, Occupational Therapy
Requirements: The program is open to students who are U.S. citizens at junior level in college or above.
Students must be enrolled in an accredited program by the appropriate health therapy profession authority in
physical therapy, occupational therapy, speech-language pathology or audiology and must be able to
demonstrate financial need. Approximately $150,000 is awarded annually.

The Physician Assistant Foundation has awarded more than 750 scholarships totaling more than
$1,000,000 to deserving PA students. Any student member of the American Academy of Physician
Assistants who is attending an ARC-PA-accredited PA program and is in the professional phase of his or
her program is eligible to apply. Scholarships are awarded based on the availability of funds provided
through contributions from AAPA members, individuals, and corporations. Previous PA Foundation
scholarship recipients are NOT eligible to apply.
This application form provides the PA Foundation Scholarship Committee with important information.
Please read each question carefully and answer each question fully. It is imperative that you meet the
established February 1 deadline. INCOMPLETE APPLICATIONS ARE INELIGIBLE FOR REVIEW.
Applications will be reviewed and judged on the basis of financial need, academic record, community
and professional activities, and future goals as a physician assistant. You will be notified in May 2006 of
the Scholarship Committee's decision. All monies awarded will be distributed in May 2006, and all
recipients will be recognized in published format at the AAPA's 34th Annual PA Conference in San
Francisco, CA.

• American Lung Association Golf Scholarship – Medical/Health Related Studies

American Lunch Association of Maine offers three $500 scholarships to eligible students involved in the game
of golf (players or employees of a golf facility), and who are pursuing further education in a medical/health
related field. These scholarships have been established by ALA-ME to recognize the golf courses who support
the Golf Privilege Card.
Criteria: Scholarships will be awarded based on academic achievement, financial need, references, review of
completed application and the applicant must be a non-tobacco user. Three $500 scholarships for graduating
Maine High School Seniors.
