Applications for loans will be made on the official form provided by the Foundation office and must be fully completed. • Loan Amounts Each student may receive one loan per academic year of a maximum of $5,000.00 .* Loans will be made directly to the student’s tuition credit at his or her osteopathic medical college. Each student receiving a loan shall begin to repay the loan one year after finishing postgraduate training**, which shall be limited to one residency. Duration of loan repayment period shall not exceed 5 years. Each student receiving a loan or loans must practice for three (3) years in the state of Washington *** commencing within thirty (30) days of completion of postgraduate training. Until such time as the loan is repaid, he/she shall maintain membership in the Washington Osteopathic Medical Association. The interest on student loans shall be 3% per annum computed from the date of graduation from college. If the student leaves school, the loan plus 10% interest must be repaid within one (1) year. If he/she fails to practice in the state of Washington or violates any of the loan requirements, the loan(s) become(s) immediately subject to repayment plus interest of 10% per annum on the unpaid balance of principal and interest computed from date loan was granted. *The loan amount will be determined by the Foundation Board of Directors. ** An approved postgraduate program which is served during obligatory military or public health service is considered on the same basis as all postgraduate progtrams in computing commencement of loan repayment. ***The Foundation may waive the requirement for practice within the State of Washington during obligatory military or public health service assignment out of the State of Washington when such service is not also part of an approved postgraduate training program. • Washington Osteopathic Foundation, Inc. Any osteopathic student, after completion of one year of osteopathic training in any of the legally accredited schools of osteopathic medicine, may apply for either or both scholarships. Applicant must be a student member of the Washington Osteopathic Medical Association. The application deadline is February 15. The Washington Osteopathic Foundation reserves the right to select scholarship recipients on the basis of Washington residency or any other criteria within its sole discretion. He spent over thirty years working for the osteopathic profession in various capacities. In addition he devoted hundreds of hours of service to the West Seattle Kiwanis Club, the West Seattle Chamber of Commerce and other local organizations. Because of this, the selection process will be based, in a major way, on the applicant’s community and human service experience. * Eugene Imamura Scholarship - $1000 Eugene Imamura, DO, retired from his Family Practice of 48 years in 2002. In recognition of
assistance he received for his medical school education, he wanted to create a scholarship fund based on need. Upon his retirement, he made a generous donation to the Washington Osteopathic Foundation to provide the seed money for a scholarship. WEST VIRGINIA • Financial Incentive Programs http://www.wvochs.org/dr/loans.aspx The Division of Recruitment offers two financial incentive programs for primary health care professionals who are interested in practicing in one of our underserved communities. The State Loan Repayment Program is funded through the Federal Bureau of Health Professions and the State and is solely for the purpose of medical educational loan repayment. The Recruitment and Retention Community Project (RRCP) is funded through the State and community and can be used for medical educational loan repayment, residency stipend, retention, sign-on bonus or other incentives the community may need to attract or keep a primary care physician, nurse practitioner, physician assistant, and/or certified nurse midwife. The RRCP cannot be used for salary supplementation. All incentive programs are subject to availability of funds. o State Loan Repayment Program (SLRP) $40,000 for a two-year commitment Two one-year extensions may be awarded for a maximum of $25,000 for each added year of service Practice site must be in a Health Professional Shortage Area (HPSA) Employer must be non-profit o Recruitment and Retention Community Project (RRCP) This program provides dollars to a community to help recruit or retain a primary care provider for the community. The sponsoring organization must provide matching funds. $20,000 maximum for a one-year commitment (50/50 match) Three one-year extensions may be awarded WISCONSIN • Health Professions Loan Assistance Program (HPLAP) http://commerce.wi.gov/CD/CD-phc.html The Health Professions Loan Assistance Program is designed to provide incentives for physicians, dentists, nurse practitioners (NPs), physician assistants (PAs) registered dental hygienists (RDHs) and certified nurse midwives (CNMs) to practice in Wisconsin rural and urban medical shortage areas. o PROGRAM BENEFITS • Physicians and dentists can receive up to $50,000* and PAs, CNMs, NPs, and RDHs can receive up to $20,000* in loan repayment over a three year period, specifically: • Physicians and dentists can receive up to $50,000 and NPs, PAs, RDHs and CNMs can receive up to $25,000 in loan repayment assistance over a three year period at the following rate: • In the first and second year, 40 percent of the principal balance or a maximum of $20,000 for physicians and dentists, and $10,000 for other professions. • In the third year, 20 percent of the principal balance or a maximum of $10,000 for physicians and dentists and $5,000 for other professions. *Award amounts can not exceed 75% of outstanding medical and school loans. o ELIGIBLE APPLICANTS • Physicians and dentists who are board-eligible or board-certified in the specialties of Family Practice, General Internal Medicine, General Pediatrics and Obstetrics can participate in the program. • NPs, PAs, RDHs and CNMs if they are licensed, have the appropriate certification and will be practicing primary care in a primary-care setting.
