student and upon maintenance <strong>of</strong> passing marks and honorable conduct without discredit <strong>to</strong> <strong>the</strong> school, <strong>the</strong><br />

pr<strong>of</strong>ession <strong>of</strong> osteopathic medicine and surgery, or <strong>the</strong> Foundation.<br />

Application deadlines are February 15, May 15, August 15 and November 15.<br />

Applications for loans will be made on <strong>the</strong> <strong>of</strong>ficial form provided by <strong>the</strong> Foundation <strong>of</strong>fice and must be<br />

fully completed.<br />

• Loan Amounts<br />

Each student may receive one loan per academic year <strong>of</strong> a maximum <strong>of</strong> $5,000.00 .* Loans will be made<br />

directly <strong>to</strong> <strong>the</strong> student’s tuition credit at his or her osteopathic medical college.<br />

Each student receiving a loan shall begin <strong>to</strong> repay <strong>the</strong> loan one year after finishing postgraduate training**,<br />

which shall be limited <strong>to</strong> one residency. Duration <strong>of</strong> loan repayment period shall not exceed 5 years.<br />

Each student receiving a loan or loans must practice for three (3) years in <strong>the</strong> state <strong>of</strong> Washing<strong>to</strong>n ***<br />

commencing within thirty (30) days <strong>of</strong> completion <strong>of</strong> postgraduate training. Until such time as <strong>the</strong> loan is<br />

repaid, he/she shall maintain membership in <strong>the</strong> Washing<strong>to</strong>n Osteopathic Medical Association.<br />

<strong>The</strong> interest on student loans shall be 3% per annum computed from <strong>the</strong> date <strong>of</strong> graduation from college. If<br />

<strong>the</strong> student leaves school, <strong>the</strong> loan plus 10% interest must be repaid within one (1) year. If he/she fails <strong>to</strong><br />

practice in <strong>the</strong> state <strong>of</strong> Washing<strong>to</strong>n or violates any <strong>of</strong> <strong>the</strong> loan requirements, <strong>the</strong> loan(s) become(s)<br />

immediately subject <strong>to</strong> repayment plus interest <strong>of</strong> 10% per annum on <strong>the</strong> unpaid balance <strong>of</strong> principal and<br />

interest computed from date loan was granted.<br />

*<strong>The</strong> loan amount will be determined by <strong>the</strong> Foundation Board <strong>of</strong> Direc<strong>to</strong>rs.<br />

** An approved postgraduate program which is served during obliga<strong>to</strong>ry military or public health service is<br />

considered on <strong>the</strong> same basis as all postgraduate progtrams in computing commencement <strong>of</strong> loan<br />

repayment.<br />

***<strong>The</strong> Foundation may waive <strong>the</strong> requirement for practice within <strong>the</strong> State <strong>of</strong> Washing<strong>to</strong>n during<br />

obliga<strong>to</strong>ry military or public health service assignment out <strong>of</strong> <strong>the</strong> State <strong>of</strong> Washing<strong>to</strong>n when such service is<br />

not also part <strong>of</strong> an approved postgraduate training program.<br />

• Washing<strong>to</strong>n Osteopathic Foundation, Inc. (WOF) Scholarship Program<br />

Each student may receive one <strong>of</strong> each scholarship. Any osteopathic student, after completion <strong>of</strong> one year <strong>of</strong><br />

osteopathic training in any <strong>of</strong> <strong>the</strong> legally accredited schools <strong>of</strong> osteopathic medicine, may apply for ei<strong>the</strong>r or both<br />

<strong>scholarships</strong>. Applicant must be a student member <strong>of</strong> <strong>the</strong> Washing<strong>to</strong>n Osteopathic Medical Association. <strong>The</strong><br />

application deadline is February 15.<br />

<strong>The</strong> Washing<strong>to</strong>n Osteopathic Foundation reserves <strong>the</strong> right <strong>to</strong> select scholarship recipients on <strong>the</strong> basis <strong>of</strong><br />

Washing<strong>to</strong>n residency or any o<strong>the</strong>r criteria within its sole discretion.<br />

<strong>The</strong> grant <strong>of</strong> any scholarship is conditional upon faithful attendance at classes and performance <strong>of</strong> duties as a student<br />

and upon maintenance <strong>of</strong> passing marks and honorable conduct without discredit <strong>to</strong> <strong>the</strong> school, <strong>the</strong> pr<strong>of</strong>ession <strong>of</strong><br />

osteopathic medicine or <strong>the</strong> Foundation.<br />

Applicant must be <strong>of</strong> legal age or <strong>have</strong> a co-signer <strong>of</strong> legal age <strong>to</strong> execute a promissory note Foundation.<br />

* Warren Lawless Scholarship - $1000<br />

Warren Lawless, former Executive Direc<strong>to</strong>r <strong>of</strong> <strong>the</strong> Washing<strong>to</strong>n Osteopathic Medical Association<br />

and current Chairman <strong>of</strong> <strong>the</strong> Board <strong>of</strong> Western <strong>University</strong> <strong>of</strong> Health Sciences, is a strong believer<br />

in community service. He spent over thirty years working for <strong>the</strong> osteopathic pr<strong>of</strong>ession in<br />

various capacities. In addition he devoted hundreds <strong>of</strong> hours <strong>of</strong> service <strong>to</strong> <strong>the</strong> West Seattle<br />

Kiwanis Club, <strong>the</strong> West Seattle Chamber <strong>of</strong> Commerce and o<strong>the</strong>r local organizations. Because <strong>of</strong><br />

this, <strong>the</strong> selection process will be based, in a major way, on <strong>the</strong> applicant’s community and human<br />

service experience.<br />

* Eugene Imamura Scholarship - $1000<br />
