In return for financial assistance, participants in both programs agree <strong>to</strong> provide primary care health care in rural or underserved urban<br />

areas with designated shortages for a minimum <strong>of</strong> three years.<br />

o Loan Repayment<br />

Eligibility <strong>to</strong> Apply<br />

You must:<br />

• Be employed or <strong>have</strong> a contract <strong>to</strong> begin employment at an eligible site.<br />

• Submit documentation <strong>of</strong> eligible outstanding educational debt (does not include Parent Plus loans or loans<br />

not in your name).<br />

• Be a United States citizen.<br />

• Provide primary care and not specialty care.<br />

• Sign a contract <strong>to</strong> serve for a minimum <strong>of</strong> three years or pay funds back at double penalty.<br />

Award<br />

<strong>The</strong> Loan Repayment program provides up <strong>to</strong> $25,000 per year for a minimum <strong>of</strong> three years. <strong>The</strong> service obligation<br />

period is based on full time (40 or more hours per week) employment and is prorated for less than full time. <strong>The</strong><br />

award amount is based on your outstanding educational debt. <strong>The</strong>re is an opportunity <strong>to</strong> apply for a fourth and fifth<br />

year for up <strong>to</strong> $35,000 per year based on remaining eligible debt.<br />

<strong>The</strong> award period is divided in<strong>to</strong> quarters. Credit is earned during <strong>the</strong> quarter and paid at <strong>the</strong> end <strong>of</strong> each quarter.<br />

Those funds must be fully and directly applied <strong>to</strong> <strong>the</strong> approved lender(s) identified in <strong>the</strong> award notification.<br />

Eligibility – You must:<br />

• Be a student training <strong>to</strong> become a primary care health pr<strong>of</strong>essional in an eligible pr<strong>of</strong>ession.<br />

• Be a United States citizen.<br />

• Have completed all prerequisite course work.<br />

• Not be in default on any educational loans.<br />

• Sign a Promissory Note agreeing <strong>to</strong> serve for a minimum <strong>of</strong> three years in a designated shortage area in<br />

Washing<strong>to</strong>n state or pay back funds at double penalty plus interest.<br />

Award<br />

Scholarship program awards vary by educational program. <strong>The</strong> scholarship is not based on financial need and you<br />

do not <strong>have</strong> <strong>to</strong> attend a Washing<strong>to</strong>n state school. You do not <strong>have</strong> <strong>to</strong> be a Washing<strong>to</strong>n state resident <strong>to</strong> apply.<br />

Upon completion <strong>of</strong> your academic program:<br />

• You will <strong>have</strong> a nine month grace period <strong>to</strong> find employment that meets <strong>the</strong> program criteria. Exception,<br />

medical students do not <strong>have</strong> a grace period upon completion <strong>of</strong> <strong>the</strong>ir residency program.<br />

• You will <strong>have</strong> a three year minimum service obligation in Washing<strong>to</strong>n state. <strong>The</strong> service obligation period<br />

is based on full time (40 or more hours per week) employment and is prorated for less than full time.<br />

• You will be required <strong>to</strong> return service confirmation forms at <strong>the</strong> end <strong>of</strong> each quarter.<br />

How <strong>to</strong> Apply<br />

Scholarship program applications are available in January and due in April.<br />

• Eligibility<br />

Any osteopathic student, after completion <strong>of</strong> six months <strong>of</strong> Osteopathic training in any <strong>of</strong> <strong>the</strong> legally<br />

accredited schools <strong>of</strong> Osteopathic Medicine and Surgery, may apply for a loan. Applicant must be a student<br />

member <strong>of</strong> <strong>the</strong> Washing<strong>to</strong>n Osteopathic Medical Association.<br />

Applicant must be <strong>of</strong> legal age or <strong>have</strong> a co-signer <strong>of</strong> legal age <strong>to</strong> execute a promissory note. <strong>The</strong><br />

Washing<strong>to</strong>n Osteopathic Foundation reserves <strong>the</strong> right <strong>to</strong> select loan recipients on <strong>the</strong> basis <strong>of</strong> Washing<strong>to</strong>n<br />

residency or any o<strong>the</strong>r criteria within its sole discretion.<br />
