<strong>The</strong> purpose <strong>of</strong> <strong>the</strong> program is <strong>to</strong> recruit and retain qualified physicians <strong>to</strong> provide primary health care in areas <strong>of</strong> Texas that<br />

are medically under-served and for certain state agencies. Applicants meeting additional requirements may be eligible for<br />

federal matching funds through <strong>the</strong> State Loan Repayment Program.<br />

Eligibility Requirement:<br />

Physicians must:<br />

o Have a current unrestricted license with no disciplinary action from <strong>the</strong> Texas Medical Board;<br />

o Practice an approved specialty;<br />

o Work at an approved practice site;<br />

o Have earned board certification within <strong>the</strong> last six years in one <strong>of</strong> <strong>the</strong> approved specialties, or (except for general<br />

practitioners) <strong>have</strong> satisfac<strong>to</strong>rily completed an approved postgraduate program;<br />

o Accept Medicaid assignment as full payment for services, and not deny services based on a patient's ability <strong>to</strong> pay;<br />

o Not <strong>have</strong> received start-up assistance under Chapter 46, <strong>of</strong> <strong>the</strong> Health and Safety Code<br />

Approved Specialties:<br />

Family Practice, Osteopathic Family Practice, Obstetrics/Gynecology, General Internal Medicine, General Pediatrics,<br />

Psychiatry, Emergency Medicine* and General Surgery*.<br />

*Emergency Medicine and General Surgery are not eligible for matching State Loan Repayment Program funds.<br />

Service Period:<br />

<strong>The</strong> physician must provide eligible services for at least 12 consecutive months at an approved practice site. To qualify for<br />

federal matching funds applicants must provide eligible services for at least 24 consecutive months at an approved practice<br />

site. <strong>The</strong> first year <strong>of</strong> service is for 12 consecutive months from <strong>the</strong> date <strong>the</strong> application is received or <strong>the</strong> date eligible service<br />

began, whichever is later. Subsequent years begin at <strong>the</strong> end <strong>of</strong> <strong>the</strong> previous service period.<br />

Annual Loan Amounts:<br />

<strong>The</strong> maximum annual state funded loan repayment for an eligible physician providing full-time service is $9,000. Applicants<br />

qualifying for federal matching funds may receive up <strong>to</strong> $18,000 after 24 months <strong>of</strong> consecutive service and 12 months <strong>of</strong><br />

continuous service for subsequent years. Repayments are contingent on availability <strong>of</strong> funds.<br />

<strong>The</strong> Utah Health Care Workforce Financial Assistance Program was created <strong>to</strong> provide pr<strong>of</strong>essional education loan<br />

repayment assistance <strong>to</strong> health care pr<strong>of</strong>essionals who locate or continue <strong>to</strong> practice in underserved areas <strong>of</strong> Utah. A<br />

minimum two (2) year service commitment at an employment site approved by <strong>the</strong> Department, is required by those<br />

individuals that are awarded educational loan repayment funding.<br />

Health care pr<strong>of</strong>essionals eligible for this Program include <strong>the</strong> <strong>following</strong> "fully licensed" health care pr<strong>of</strong>essionals:<br />

• Dentist (D.D.S. and/or D.M.D.)<br />

• Non-Physician Mental Health <strong>The</strong>rapist (clinical psychologists, licensed clinical social workers, licensed<br />

pr<strong>of</strong>essional counselors, and/or marriage and family <strong>the</strong>rapists)<br />

• Nurse (L.P.N. through Ph.D. - who received <strong>the</strong>ir degree from a college/school <strong>of</strong> nursing)<br />

• Physician (D.O. and/or M.D.)<br />

• Physician Assistant (must <strong>have</strong> passed <strong>the</strong> National Certification Examination in order <strong>to</strong> be eligible)
