The purpose of the program is to recruit and retain qualified physicians to provide primary health care in areas of Texas that
are medically under-served and for certain state agencies. Applicants meeting additional requirements may be eligible for
federal matching funds through the State Loan Repayment Program.
Eligibility Requirement:
Physicians must:
o Have a current unrestricted license with no disciplinary action from the Texas Medical Board;
o Practice an approved specialty;
o Work at an approved practice site;
o Have earned board certification within the last six years in one of the approved specialties, or (except for general
practitioners) have satisfactorily completed an approved postgraduate program;
o Accept Medicaid assignment as full payment for services, and not deny services based on a patient's ability to pay;
o Not have received start-up assistance under Chapter 46, of the Health and Safety Code
Approved Specialties:
Family Practice, Osteopathic Family Practice, Obstetrics/Gynecology, General Internal Medicine, General Pediatrics,
Psychiatry, Emergency Medicine* and General Surgery*.
*Emergency Medicine and General Surgery are not eligible for matching State Loan Repayment Program funds.
Service Period:
The physician must provide eligible services for at least 12 consecutive months at an approved practice site. To qualify for
federal matching funds applicants must provide eligible services for at least 24 consecutive months at an approved practice
site. The first year of service is for 12 consecutive months from the date the application is received or the date eligible service
began, whichever is later. Subsequent years begin at the end of the previous service period.
Annual Loan Amounts:
The maximum annual state funded loan repayment for an eligible physician providing full-time service is $9,000. Applicants
qualifying for federal matching funds may receive up to $18,000 after 24 months of consecutive service and 12 months of
continuous service for subsequent years. Repayments are contingent on availability of funds.

The Utah Health Care Workforce Financial Assistance Program was created to provide professional education loan
repayment assistance to health care professionals who locate or continue to practice in underserved areas of Utah. A
minimum two (2) year service commitment at an employment site approved by the Department, is required by those
individuals that are awarded educational loan repayment funding.
Health care professionals eligible for this Program include the following "fully licensed" health care professionals:
• Dentist (D.D.S. and/or D.M.D.)
• Non-Physician Mental Health Therapist (clinical psychologists, licensed clinical social workers, licensed
professional counselors, and/or marriage and family therapists)
• Nurse (L.P.N. through Ph.D. - who received their degree from a college/school of nursing)
• Physician (D.O. and/or M.D.)
• Physician Assistant (must have passed the National Certification Examination in order to be eligible)
