Recent data from the NHANES III and the Framingham Heart Study in the USA suggest that mortality and cardiovascular events increase in line with increasing doses of serum 25-hydroxyvitamin D levels above 40 ng/mL. Epidemiological studies show that an increasing trend in pancreas cancer incidence with high vitamin D status. Vitamin D requirement, supplementation situations, advantages and disadvantages of vitamin D supplementation will be discussed in this section. GAG’ lar, doku, hücre ve fibröz dokuların kararlılığını sağlamanın yanı sıra, vücudun su ve tuz dengesini de sağlarlar. Yedi tip GAG monomeri içerdikleri glikozid bağı tipi ve sülfat birimlerinin yer ve derecesine göre sınıflandırılmıştır. Bunlar: kondroitin sülfat, keratan sülfat I ve II, hyaluronik asit, dermatan sülfat, heparan sülfat ve heparindir. Tıpta GAG’ların, özellikle osteoartrit tedavisinde ağrının azaltılmasında yaygın kullanımları vardır. Osteoartrit dışında, keratokonjuktivit ve psöriazis tedavisinde ve kronik sistitlerde intravezikal instilasyon için kullanılmaktadır. Osteoartrit tedavisinde, glikozamin ve/veya kondroitin sülfat desteği, oral olarak veya daha az oranda topikal olarak kullanılmaktadır. Hyaluronik asitin eklem içi uygulamaları da yapılabilmektedir. Osteoartrit semptomlarını azaltmada ve hastalık modifiye edici potansiyeli değerlendirmede üzerinde en çok çalışılan GAG, glikozamin takviyesidir. Ancak, literatürde glikozaminin tek başına ya da kombine olarak kondroitin sülfat ile birlikte kullanıldığı çalışmalarda, semptomları giderme ve eklem aralığındaki daralma sürecinin yavaşlatılması açısından birbirinden farklı sonuçlar bulunmaktadır. Özellikle oral olarak kullanılan preperatlar, bir medikal ilaçtan çok gıda takviyesi olarak görülmektedir. Bu tür ürünlere ait tam bir yasal düzenleme bulunmamaktadır. Hekimlerin bir kısmı tarafından tavsiye edilse de birçok hekim bu ürünlere şüpheli yaklaşmaktadır. Glycosaminoglycans (GAGs) are complexes that contain small amount of protein and long unbranched negatively charged heteropolysaccharides consisting of a repeating disacharide unit (hexuronic acid, hexosamine). GAGs provide the stability of tissues, cells and fibrous tissues as well as the balance of salt and water. Seven types of GAGs monomers were classified according to type of glycoside bond and the degree and place of sulfate units. These are chondroitin sulfate, keratan sulfate I and II, hyaluronic acid, dermatan sulfate, heparan sulfate and heparin. In medicine, GAGs are commonly used especially in reducing pain in patients with osteoarthritis. Chondroitin sulfate is used for the treatment of keratoconjuctivitis and psoriasis and for intravesical instillation in chronic cystitis. Glucasamine and/ or chondroitin sulfate supplementation is generally used orally but sometimes topically. And also, hyaluronic acid can be applied intra-articularly. Glucosamine is most studied GAGs in reducing the symtoms of osteoarthritis and in evaluation of modification potential of the disease. In literature there are conflicting results concerning the effects of glycosamine in release of symptoms and narration of joint space alone or in combination with condroitin sulfate. Especially, orally used preparates are evaluated as food support more than a medical drug. There is no legal regulation for such products.
