CELL LUNG CANCER IN AEGEAN PART OF TURKEY<br />

Akciğeri kanseri olgularının büyük bir kısmı küçük hücreli dışı akciğer kanserinden<br />
(KHDAK) oluşmaktadır. Son yıllarda, biyomarker kökenli kişiselleştirilmiş tıptaki<br />
ilerleme KHDAK’nin epidermal büyüme faktör reseptörü (EGFR) – tirozin kinaz<br />
birimini hedefleyen erlotinib ve gefitinib terapileriyle yapılan tedavi yaklaşımını<br />
değiştirmiştir. Bu çalışmanın amacı enstitümüzde tedavi edilen KHDAK hastaları<br />
arasında EGFR gen mutasyonlarının sıklığını belirlemektir. KHDAK’li toplam 78<br />
hasta, EGFR geni ekzon 18, 19, 20 ve 21 bölgelerindeki mutasyonlar açısından<br />
otomatize mikroarray platformu (INFINITI® AutoGenomics, Inc.) kullanılarak<br />
değerlendirildi. 23 hastada (2%9.48) EGFR mutasyonları saptandı: Tüm kadın<br />
olguların %35.71’i ve tüm erkek olguların %28.12’sinde mutasyon saptandı.<br />
Ekzon 18, 19, 20 ve 21 için EGFR mutasyon sıklığı sırasıyla %1.28, %10.25,<br />
%14.10 ve %5.33 olarak saptandı. Sonuçlarımız Avrupa ve Asya raporlarıyla<br />
kıyaslandığında EGFR için farklı mutasyon eğilimi göstermektedir. KHDAK’li<br />
grubumuzdaki mutasyon sıklığı (% 29.48) Batı Asya hastalarının EGFR mutasyon<br />
sıklığı ile (%40 ile %64 arasında) yakın, fakat Avrupa hastalarına (% 10 ile<br />
%16.6 arasında) yakın değildir. Literatürle benzer şekilde grubumuzda EGFR<br />
mutasyonları kadınlarda erkeklere göre daha sıktır. EGFR mutasyon sıklığı sadece<br />
etnisite ile değişiklik göstermez, aynı zamanda cinsiyet, sigara kullanım durumu<br />
ve KHDAK’nin histolojik tipi ile de değişiklik gösterir. Kanser tedavisiyle ilişkili<br />
moleküler özellikler KHDAK’nin tanı ve terapötik süreçlerinin yönetimi için<br />
önemlidir.<br />
Non–small cell lung cancer (NSCLC) accounts for the majority of lung cancer<br />
cases. In recent years, the advancement of biomarker-driven personalized therapy<br />
has changed an approach to the treatment of NSCLC with epidermal growth<br />
factor receptor (EGFR) tyrosine kinase domain target-directed therapies erlotinib<br />
and gefitinib. The aim of this study was to examine the frequency of EGFR gene<br />
mutations among patients with NSCLC treated in our institution. A total of 78<br />
patients with NSCLC were examined for EGFR mutations in exons 18, 19, 20 and<br />
21 using an automated microarray platform (INFINITI® AutoGenomics, Inc.).<br />
EGFR mutations were detected in 23 patients (29.48%): 35.71% of all female<br />
cases and 28.12% of all male cases had mutations. EGFR mutations frequencies<br />
determined for exon 18, 19, 20 and 21 were 1.28%, 10.25%, 14.10% and 5.33%,<br />
respectively. Our results showed different mutational trends when compared<br />
to European and Asian reports. The frequency of EGFR mutations in our group<br />
(29.48%) with NSCLC was found to be closer to East Asian patients (between<br />
40% to 64%), but not similar to European patients (between 10% to 16.6%).<br />
In our group, EGFR mutations were more frequent in women than men similar<br />
to the literature. The frequency of EGFR mutations varies not only with the<br />
ethnicity, but also with gender, smoking status, and histologic type of NSCLC.<br />
Molecular features associated with cancer treatment is important for management<br />
of diagnostic and therapeutic processes in NSCLC.<br />
