Step 3: Case <strong>Plan</strong> adapted to include parent/child visitation.<br />

ACCOMPLISHMENTS:<br />

1 st Quarter Report: The Case <strong>Plan</strong> has been amended to include documentation <strong>of</strong> visits between parents and children. This is<br />

currently being covered in the CPRS training, which will be complete at the end <strong>of</strong> January 2003. All staff is required to document<br />

ef<strong>for</strong>ts to place siblings together.<br />

2 nd Quarter Report: No Report due this quarter.<br />

3 rd Quarter Report: ACHIEVED. See attached CPRS printouts. The Case <strong>Plan</strong> and Reporting System (CPRS) has been modified to<br />

capture documentation <strong>of</strong> visits between parents and children. This system currently captures who visits the child and where the visits<br />

occur. Ef<strong>for</strong>ts are in progress to improve the system so that the choice <strong>of</strong> visitation sites (e.g., the least restrictive setting possible) will,<br />

also, be captured.<br />

Concurrent with Case <strong>Plan</strong> modifications regarding visitations, discussed above, are on-going ef<strong>for</strong>ts between C&S staff and local<br />

<strong>of</strong>fice staff. Their ef<strong>for</strong>ts assure that case plans: 1) identify time frames <strong>for</strong> the visits; 2) identify specific locations where the visits will<br />

take place; 3) document family interaction during visits; 4) specify objectives <strong>of</strong> the visits; and 5) documents who should be notified if<br />

visits change.<br />

Step 4: Caseworkers are trained to include documentation <strong>of</strong> visits in case plan.<br />
