Ef<strong>for</strong>ts to place siblings in the same home will be<br />

documented in at least 90% <strong>of</strong> cases by June 2004.<br />

ACCOMPLISHMENTS:<br />

1 st Quarter Report: Goal 2, Steps 1 - 4: A workgroup is being <strong>for</strong>med to address changes needed to improve the Supervisory Review<br />

Form and implementation process. The committee is aware <strong>of</strong> PIP requirements to place siblings together, if possible or feasible. They<br />

expect to revise the Supervisory Review Form to document ef<strong>for</strong>ts to place siblings together.<br />

2 nd Quarter Report: Achieved. The requirements <strong>for</strong> exhaustive ef<strong>for</strong>ts to place sibling groups together were emphasized in CPRS<br />

training sessions completed in February 2003 <strong>for</strong> staff statewide. All staff are required to document ef<strong>for</strong>ts to place siblings together.<br />

2 nd Quarter Federal Response: What can the State give us to show this has been achieved?<br />

Georgia's Reply 3 rd Quarter: The state respectfully requests to change the 2 nd Quarter Report from 'Achieved' to reflect, 'Partially<br />

Achieved'. This request is being made because, while all supervisors were trained to document ef<strong>for</strong>ts to place siblings together on the<br />

CPRS, the actual Supervisory Review <strong>for</strong>m (stipulated under the Measurable Benchmarks) is still in process <strong>of</strong> revision.<br />

3 rd Quarter Federal Response: OK.<br />

3 rd Quarter Report: Partially Achieved. Ef<strong>for</strong>ts to revise the Supervisory Review Form continue as indicated above, under Goal # 2,<br />

DOCUMENTATION:<br />

Please see attachment CPRS Screen printout, Permanency 2, Item 12, which confirms ef<strong>for</strong>ts to place siblings together are properly<br />

documented in case files. Also, see attachments, CPRS v2, Trained by County & Region -- Detail and PRS v2, Trained by Training<br />

Date -- Detail, which show that staff from all 159 Georgia counties have been trained to include ef<strong>for</strong>ts to place siblings together in the<br />

case plan.<br />

5th Quarter Report: Partially Achieved. The state has made the decision to discontinue using the Supervisory Review Guide and<br />

replaces it with the Child Placement Services E & R Social Services Review Guide, developed by the Evaluation and Reporting (E &<br />
