contract requires the Regional Advisors to distribute brochures on the Center's services to every County DFCS <strong>of</strong>fice. This<br />

in<strong>for</strong>mation includes a post adoption services listing and an adoption assistance handbook. In addition, the advisors ask to attend the<br />

9 th session <strong>of</strong> MAPP so they may discuss the resources available to families. Each prospective foster and adoptive parent is given a<br />

Resource and Adoption Assistance Directory at MAPP, when the Form 150 is signed or when the Placement Agreement is signed. The<br />

county, by policy, is to have the family sign a Form 399 identifying they have received this material and <strong>for</strong>ward the <strong>for</strong>m to the<br />

Office <strong>of</strong> Adoptions. To date we have received 324 <strong>for</strong>ms. En<strong>for</strong>cement <strong>of</strong> this policy continues to be a training issue that will be<br />

emphasized by the Regional Adoption Coordinators with the County DFCS <strong>of</strong>fices.<br />

BARRIERS TO ACHIEVEMENT:<br />

E27 ACTION STEPS: At time <strong>of</strong> signing <strong>of</strong> the Form 33/37 Placement Agreement, provide a packet <strong>of</strong> in<strong>for</strong>mation from the Georgia<br />

Center <strong>for</strong> Adoption Resources and Support that will assist families in identifying post adopt services, support<br />

groups, community resources and events relating to adoption. Packets will include copies <strong>of</strong> the Adoption<br />

Assistance Handbook.<br />

ACCOMPLISHMENTS:<br />

1 st 2 nd and 3 rd Quarter Reports: Achieved. Each region in the state has been provided with the number <strong>of</strong> packages they requested to<br />

satisfy accomplishing this step. They signed <strong>for</strong> the packets and the signed copies were given to the Regional Advisors <strong>for</strong> the Center<br />

<strong>for</strong> Adoption Resources and Support. A letter was sent to each county in August 2002 regarding distribution to all prospective foster<br />

and adoptive parents at: MAPP; the signing <strong>of</strong> the <strong>for</strong>m 150; or at the time <strong>of</strong> the signing <strong>of</strong> the adoptive placement agreement.<br />

Verification that the family received this in<strong>for</strong>mation is required. Each packet is to be signed by the recipient and the <strong>for</strong>m sent to the<br />

Office <strong>of</strong> Adoptions.<br />
