Held within 30 – 60 days <strong>of</strong> a child entering foster care,<br />

Comprised <strong>of</strong> pr<strong>of</strong>essionals from diverse disciplines (assessment provider, mental health, public health, school counselors, CASA,<br />

physici<strong>an</strong>s, psychologist, psychiatrist, resource parents, <strong>an</strong>d DFCS staff),<br />

Considered as one <strong>of</strong> the foundations <strong>of</strong> the FP/BP process,<br />

Considered the ultimate in public/private collaboration.<br />

The team comes together at the conclusion <strong>of</strong> the Comprehensive Child <strong>an</strong>d Family Assessment process to:<br />

Review information from the assessment,<br />

Provide consultation,<br />

Promote coordination between agencies.<br />

The team will make the best <strong>an</strong>d most appropriate recommendations for:<br />

Services<br />

Placements that meet the needs <strong>of</strong> the child <strong>an</strong>d family.<br />

Possible outcomes <strong>of</strong> the MDT meeting:<br />

Provide a checks <strong>an</strong>d bal<strong>an</strong>ces mech<strong>an</strong>ism to ensure that the child <strong>an</strong>d families strengths <strong>an</strong>d needs are addressed appropriately,<br />

Identify service gaps <strong>an</strong>d breakdowns in coordination or communication between agencies or individuals.<br />

The birth parents <strong>an</strong>d relatives are invited to attend the MDT meeting. However they do not count as a discipline.<br />

An appropriate MDT meeting must have at a minimum three (3) disciplines represented.<br />

The enh<strong>an</strong>cements to the CPRS system provide the state with the capability to use the data entered by the FP/BP provider to determine<br />

if MDT meetings are held <strong>an</strong>d to identify the recommendations regarding health care needs <strong>an</strong>d services for children in foster care.<br />
