Please see attachment CPRS Screen printout, Perm<strong>an</strong>ency 2, Item 12, which confirms efforts to place siblings together are properly<br />

documented in case files. Also, see attachments, CPRS v2, Trained by County & Region -- Detail <strong>an</strong>d PRS v2, Trained by Training<br />

Date -- Detail, which show that staff from all 159 Georgia counties have been trained to include efforts to place siblings together in the<br />

case pl<strong>an</strong>.<br />

5th Quarter Report: Partially Achieved. The state has made the decision to discontinue using the Supervisory Review Guide <strong>an</strong>d<br />

replaces it with the Child Placement Services E & R Social Services Review Guide, developed by the Evaluation <strong>an</strong>d Reporting (E &<br />

R) <strong>of</strong>fice. This instrument is already developed <strong>an</strong>d available to the staff. Also, it incorporates PIP requirements for Georgia. It<br />

encompasses the same information contained in the previous Supervisory Review Guide, <strong>an</strong>d includes a reference to efforts to place<br />

siblings together (under Item 28). The E & R Review Guide is already available to staff <strong>an</strong>d requires no additional training. According<br />

to the first round <strong>of</strong> QCRs efforts to place siblings together was documented in 10 <strong>of</strong> the 13 applicable cases. See the attached<br />

Qualitative Review Report for October 2003 -- December2003.<br />

6 th Quarter Report: The most recent Qualitative Case Review for J<strong>an</strong>uary 2004 – March 2004 indicates <strong>an</strong> increase in the number <strong>of</strong><br />

siblings placed together. Of 37 cases examined, 23 were relev<strong>an</strong>t to the placement <strong>of</strong> siblings together. Documentation <strong>of</strong> efforts to<br />

place siblings together was indicated in 21 cases (91.30%).<br />

7 th Quarter Report: ACHIEVED. This item was achieved with the previous report. Our failure to indicate it was <strong>an</strong> oversight.<br />

Supervisors have been trained to include efforts to place siblings together in their reviews. Training will continue to be extended to<br />

supervisors as needed or when updates/<strong>improvement</strong>s to the system are made.<br />

7 th Quarter DOCUMENTATION: A cumulative Qualitative Case Review for the 10 month period 10/03 – 06/04 shows that in 45<br />

relev<strong>an</strong>t cases, 38 cases (84.44%) were rated as a “strength”. This item was rated “needs <strong>improvement</strong>” in seven cases (15.56%).<br />

Forty cases involved siblings where, if appropriate, the agency should have made attempts to keep siblings together. The report<br />

indicates that the agency made diligent attempts to keep siblings together in 34 <strong>of</strong> the 40 cases (85.00%).<br />

8 th Quarter Report: Achieved. As reported in the 6 th quarter report, Georgia’s Quality Case Review (QCR) indicates that efforts to<br />

place siblings together were documented in 91.30% <strong>of</strong> the cases reviewed. The federal final CFSR report indicates that 72.2% <strong>of</strong><br />

cases document efforts to place siblings together. The QCR for 10/2003 – 09/2004 implies that in 92.6% <strong>of</strong> the applicable cases,<br />

diligent efforts were made to place siblings together.<br />
