All placement supervisors will be trained on the revised<br />

Supervisory Review form <strong>an</strong>d st<strong>an</strong>dards to document<br />

efforts to place siblings together.<br />

Efforts to place siblings in the same home will be<br />

documented in at least 90% <strong>of</strong> cases by June 2004.<br />

1 st Quarter Report: Goal 2, Steps 1 - 4: A workgroup is being formed to address ch<strong>an</strong>ges needed to improve the Supervisory Review<br />

Form <strong>an</strong>d implementation process. The committee is aware <strong>of</strong> PIP requirements to place siblings together, if possible or feasible. They<br />

expect to revise the Supervisory Review Form to document efforts to place siblings together.<br />

2 nd Quarter Report: Achieved. The requirements for exhaustive efforts to place sibling groups together were emphasized in CPRS<br />

training sessions completed in February 2003 for staff statewide. All staff are required to document efforts to place siblings together.<br />

2 nd Quarter Federal Response: What c<strong>an</strong> the State give us to show this has been achieved?<br />

Georgia's Reply 3 rd Quarter: The state respectfully requests to ch<strong>an</strong>ge the 2 nd Quarter Report from 'Achieved' to reflect, 'Partially<br />

Achieved'. This request is being made because, while all supervisors were trained to document efforts to place siblings together on the<br />

CPRS, the actual Supervisory Review form (stipulated under the Measurable Benchmarks) is still in process <strong>of</strong> revision.<br />

3 rd Quarter Federal Response: OK.<br />

3 rd Quarter Report: Partially Achieved. Efforts to revise the Supervisory Review Form continue as indicated above, under Goal # 2,<br />
