Session Descriptions Accident Investigation Fact: In most workplaces, accident investigation procedures are not as effective as they can be. The cause: Poor design and performance of the accident investigation plans. The solution: Application of the concepts and principles presented in this workshop. Safety committees required by OAR 437- 001-0765(6)(g) to establish procedures for investigating incidents and accidents. This workshop introduces participants to a proactive six-step event analysis and system evaluation that emphasizes fact-finding, surface-and-root-cause analysis, and safety management system evaluation.
