A withdrawal made after the dates set out above will result in<br />

a fail being recorded on the student's academic record (the<br />

symbol NWD -failure because <strong>of</strong> late withdrawal -will<br />

appear).<br />

A student who believes that the failing result NWD should not<br />

be recorded must obtain the specific approval <strong>of</strong> the Pro Vice-<br />

Chancellor <strong>of</strong> the division concerned, and the Academic<br />

Registrar.<br />

Circumstances supporting the application must be set out on<br />

the Amendment to Enrotmentform on which the approval for<br />

the withdrawal is sought. A late fee <strong>of</strong> $10.00 per subject may<br />

be imposed.<br />

If, as a consequence <strong>of</strong> withdrawing from a subject or subjects,<br />

a student changes from full-time to part-time status, a refund<br />

<strong>of</strong> a portion <strong>of</strong> the general service fee will be made only if the<br />

withdrawal is made prior to 31 March 1995 for semester one,<br />

or for semester two, prior to 31 August 1995.<br />

HECS liability<br />

Students who withdraw from subjects or total enrolment after<br />

31 March 1995 for subjects concluding at the end <strong>of</strong> the first<br />

semester or after 31 August 1995 for subjects concluding at<br />

the end <strong>of</strong> the second semester will still incur a HECS liability<br />

for that semester.<br />

Students who withdraw from a full year subject after 31 March<br />

1995 will still incur a HECS liability for semester 1. Students<br />

who withdraw from a full year subject after 31 August 1995<br />

will incur a HECS liability for two semesters.<br />

Adding subjects<br />

No subject may be added to a student's enrolment without the<br />

approval <strong>of</strong> both the teaching and the awarding schools.<br />

Students should be aware that some divisions have restrictions<br />

on the period during which subjects can be added.<br />

Notwithstanding any divisional rules, after 31 March 1995 (for<br />

subjects concluding at the end <strong>of</strong> the first semester) or 31<br />

August 1995 (for subjects concluding at the end <strong>of</strong> the second<br />

semester) an amendment will be permitted only where special<br />

circumstances exist and the approval <strong>of</strong> the Pro Vice-<br />

Chancellor <strong>of</strong> the division concerned and the Academic<br />

Registrar has been given. A fee <strong>of</strong> $1 0.00 per subject will be<br />

charged. Students not enrolled in a subject during the<br />

examination period must seek approval <strong>of</strong> the division<br />

concerned. A fee <strong>of</strong> $50.00 per additional subject added will<br />

be charged.<br />

Students should <strong>note</strong> that the addition <strong>of</strong> subjects may result in<br />

a change from part-time to full-time status. In such<br />

circumstances the amendment will only be recorded when an<br />

amount <strong>of</strong> money being the difference between the part-time<br />

and full-time general service fee paid has been paid. It is the<br />

responsibility <strong>of</strong> students to ensure that they are aware <strong>of</strong> any<br />

additional fees required and to arrange for their payment at<br />

the Cashier's Office.<br />

Leave <strong>of</strong> absence<br />

Students who have enrolled in a course and who wish to apply<br />

for a period <strong>of</strong> leave <strong>of</strong> absence may do so in writing<br />

addressed to the Divisional Manager <strong>of</strong> the appropriate<br />

division. The application should clearly indicate the<br />

circumstances on which the request is based and the length <strong>of</strong><br />

time for which leave is sought.<br />

Each application is considered within the division concerned<br />

under any specific division rules relating to leave <strong>of</strong> absence.<br />

Students who have been granted leave <strong>of</strong> absence will be<br />

notified in writing by the division concerned. Enrolment for all<br />

subjects for the duration <strong>of</strong> the leave will be cancelled.<br />

Students who have been granted leave <strong>of</strong> absence will be<br />

eligible for a refund <strong>of</strong> their 1995 General Service Fee only if<br />

their application is received prior to 31 March 1995 for<br />

semester one or 31 August 1995 for semester two. Students<br />

must also attach copy <strong>of</strong> their enrolment receipt and ID card<br />

with their application. Refunds cannot be processed without a<br />

copy <strong>of</strong> the receipt and ID card.<br />

Amendments to personal<br />

details<br />

A student who changes his or her name, address or place <strong>of</strong><br />

employment should complete an Amendment to Personal<br />

Details form which is available from the Student Administration<br />

Office or the Divisional Office.<br />

Students recording a change <strong>of</strong> name will be required to<br />

produce legal documentary evidence (e.g. marriage certificate,<br />

statutory declaration, deed poll certificate) in addition to<br />

completing an Amendment to Personal Details form.<br />

Semester address for<br />

correspondence<br />

Throughout the year information regarding HECS, examination<br />

results and other special notices are sent to students.<br />

Students must ~rovide a correct address otherwise thev mav<br />

jeopardise the6 chances <strong>of</strong> meeting deadlines and obs&ving<br />

other special requirements.<br />

If a student changes an address an Amendment to Personal<br />

Details form must be completed and lodged immediately at the<br />

Divisional Office.<br />

Identity cards<br />

When on campus, all enrolled students are required to carry,<br />

and to produce on request <strong>of</strong> a member <strong>of</strong> staff, the<br />

photographic identity card issued to them.<br />

The card, which has a maximum life <strong>of</strong> four years, must be<br />

presented for updatehalidation for the forthcoming year on reenrolment.<br />

The card includes the authorisation for borrowing from the<br />

<strong>Swinburne</strong> Library.<br />

A student who loses an identity card should notify the library<br />

as soon as the loss is detected. Cardholders are, under library<br />

rules, responsible for any transaction made on the card up to<br />

the time <strong>of</strong> notification <strong>of</strong> the loss. A replacement card will be<br />

issued for a fee <strong>of</strong> $ 10.00.<br />

No refund <strong>of</strong> the general service fee will be made unless the<br />

identity card is returned to the Divisional Office with the notice<br />

withdrawal from a course.
