Fifth year medical students joined the Dermatology team for<br />

two week rotations between <strong>St</strong> Vincent’s <strong>and</strong> Hume <strong>St</strong><br />

Hospitals.<br />

Dr Collins taught on the Royal College of Physicians of Irel<strong>and</strong><br />

Membership course. Ms Sheila Ryan (CNS) taught the<br />

phototherapy nurse education programme to dermatology<br />

specialist nurses from Drogheda. Sheila also lectured third year<br />

student nurses on atopic eczema, psoriasis <strong>and</strong> skin cancer.<br />

Achievements<br />

Dr Sinead Langan (SpR 3) won the Jacob medal (Specialists<br />

Registrars prize) for her presentation entitled:Therapeutic<br />

advances in the treatment of malignant melanoma.<br />

Ms Sheila Ryan was awarded a travelling fellowship by the<br />

British Association of Dermatologists nursing group to attend<br />

the annual meeting.<br />

The Comhairle report on Dermatology services in Irel<strong>and</strong> was<br />

published in November 2003.The contributions of Dr Paul<br />

Collins,Dr John Bourke <strong>and</strong> Dr Rosemarie Watson of the<br />

Manpower subcommittee of the Irish Association of<br />

Dermatologists were acknowledged in the report.<br />

<strong>St</strong>affing<br />

2003 was an exciting <strong>and</strong> challenging time for the Emergency<br />

Department.A number of new staff joined <strong>and</strong> the department<br />

were active in audit, r e s e a r c h ,medical education <strong>and</strong> accreditation.<br />

The department continued to develop its regional approach to<br />

emergency medicine.The environment has on occasions been<br />

challenging due to overcrowding although a number of<br />

initiatives have helped to lessen the impact of overcrowding.<br />

Service Developments<br />

The consultant staff was increased to three with the<br />

appointment of Mr Derek Barton who took up his post in<br />

March. In July, Drs Jean O Sullivan <strong>and</strong> Alistair Murray started as<br />

our first Specialist Registrars.Tom Allen,one of our Clinical<br />

Fellows was appointed as a Specialist Registrar in London.<br />

The reorganisation of the waiting area in the Emergency<br />

Department was completed towards the end of 2003 with the<br />

introduction of a revamped triage room <strong>and</strong> a Rapid<br />

Assessment <strong>and</strong> Treatment Service, the latter having been<br />

piloted in December 2002.<br />

The Minor Injury Unit has been operational daily from 10:00<br />

hrs to 22:00 hrs with its own dedicated staff facilitating a<br />

streamed approach to acute healthcare delivery in the<br />

department.<br />

Close liaison with <strong>St</strong> Michael’s Hospital Respiratory Unit has<br />

allowed direct admission of respiratory cases from the<br />

Emergency Department to the <strong>St</strong> Michael’s Respiratory Unit.<br />

In April 2003 the department was challenged by the worldwide<br />

outbreak of SARS <strong>and</strong> a hospital wide protocol for the<br />

management of SARS was developed. Dr Tim Hinchy jointly<br />

won the best presentation for Medical Gr<strong>and</strong> Rounds for a<br />

presentation on SARS.<br />

Continuing professional development underpinned by the<br />

Scope of Professional Practice continues with<br />

Venepuncture <strong>and</strong> Cannulation Training,Plastering Techniques<br />

<strong>and</strong> Manchester Triage. Marianne Kelly completed an audit of<br />

the Manchester Triage practice in the Department. J Brennan, S<br />

Reynolds,J Kearns <strong>and</strong> M Kelly undertook an audit of the<br />

nursing documentation for the Emergency Department.<br />

2003 saw the first group of BSc <strong>St</strong>udent Nurses on their supernumerary<br />

specialist placement to the Emergency Department<br />

Trauma By-pass was introduced in October following extensive<br />

preparation involving all stakeholders <strong>and</strong> having been approved<br />

by the East Coast Area Health Board.A dedicated telephone<br />

<strong>and</strong> fax line were introduced to allow direct communication<br />

with ambulance crews at the scene of an accident.<br />

Accreditation<br />

The dramatic shift in emphasis from service provider to a more<br />

customer oriented health service is both socially <strong>and</strong> politically<br />

prudent. There is a real challenge for health care facilities <strong>and</strong><br />

health care professionals to provide a comprehensive high<br />

quality patient service in an ever-changing practice environment<br />

to meet patients' needs.There exists within all areas of the<br />

health services today a culture of quality management. Quality<br />

is a continuous process,<strong>and</strong>, it is only through continuous<br />

collaboration with all concerned that a quality service is<br />

achieved.Health care professionals need to explore <strong>and</strong><br />

adopt the philosophy of Continuous Quality<br />

Management in the current health care<br />
