Departmental Review<br />

Medical <strong>and</strong> Surgical Gastroenterology continues to play a<br />

major role in the activities of <strong>St</strong> Vincent’s Healthcare Group.<br />

Close to one third of all hospital admissions fit into the<br />

category of acute or chronic gastroenterology <strong>and</strong> it is<br />

appropriate that the Medical <strong>and</strong> Surgical units work so closely<br />

together. For some years now Medical <strong>and</strong> Surgical<br />

Gastroenterology have run a combined colorectal clinic,<br />

probably unique in this isl<strong>and</strong>.This clinic, plus weekly Radiology<br />

<strong>and</strong> Pathology conferences run with our Diagnostic Imaging <strong>and</strong><br />

Pathology colleagues,allow for best practice in patient care <strong>and</strong><br />

provide an excellent source of teaching for NCHDs.Further<br />

combined meetings include a monthly Audit of Colorectal<br />

Cancer <strong>and</strong> monthly research symposia in Inflammatory Bowel<br />

disease <strong>and</strong> Cancer.We wish to acknowledge the close clinical<br />

<strong>and</strong> scientific support we receive from our colleagues in<br />

Medical <strong>and</strong> Surgical Oncology <strong>and</strong> Palliative <strong>Care</strong>.We also wish<br />

to put on record our thanks to our nursing colleagues on<br />

<strong>St</strong>.Teresa’s <strong>and</strong> <strong>St</strong>. Patrick’s wards who manage our<br />

gastroenterology patients despite major involvement in general<br />

Medical <strong>and</strong> Surgical call.<br />

2003 saw the arrival of laparoscopically assisted surgery for the<br />

management of patients with Colorectal diseases.This technique<br />

established <strong>and</strong> audited by Mr. John Hyl<strong>and</strong> <strong>and</strong> his team is a<br />

major advance in the management of patients with colorectal<br />

disorders<br />

2003 has been a time of great change in <strong>St</strong> Vincent’s University<br />

Hospital.The new building that commenced in 2002 has rapidly<br />

taken shape <strong>and</strong> much of the efforts of the staff from the<br />

Centre for Colorectal Disease have been directed towards the<br />

future to ensure a successful transition to the new building.The<br />

arrival of Mr. Richard Marshall to the post of CNM2 in the<br />

endoscopy unit has been particularly opportune. His<br />

management skills have already borne fruit within the existing<br />

endoscopy unit <strong>and</strong> we look forward to his leading us into the<br />

specially designed 3- roomed Endoscopy Unit in the new<br />

building.As usual, Ms Nuala Donnelly <strong>and</strong> staff in <strong>St</strong>. Mark’s Day<br />

ward continue to appear to produce beds out of nothing for<br />

our patients <strong>and</strong> the exp<strong>and</strong>ed medical <strong>and</strong> surgical day care<br />

unit may provide some relief to the ever-present problem of<br />

finding day care beds.<br />

As the physical structure of the hospital is changing,so is the<br />

structure <strong>and</strong> nature of the clinical <strong>and</strong> basic research that is<br />

being conducted by the Centre for Colorectal Disease.The<br />

most significant event for the Centre in 2003 was the<br />

appointment of Dr Jacintha O’Sullivan as Senior Research<br />

Scientist.Dr O’Sullivan,a UCD graduate, spent a number of<br />

years in prestigious US institutions before returning in May 2003<br />

to lead our ambitious clinical <strong>and</strong> basic science research<br />

programme. Her special interests include the relationship<br />

between inflammation <strong>and</strong> cancer <strong>and</strong> we have already seen<br />

the benefits of her knowledge <strong>and</strong> enthusiasm in increased<br />

grant support, increased clinical <strong>and</strong> basic scientific staff <strong>and</strong><br />

increased output of quality research. D r. O ’ S u l l i v a n ’s org a n i s a t i o n a l<br />

skills have also allowed us to commence a number of complex,<br />

though interr e l a t e d ,colorectal cancer projects in areas such as<br />

pre-neoplastic marke rs of colorectal disease, biomarkers in early<br />

cancer <strong>and</strong> factors associated with disease in young adult<br />

patients.These projects will continue to develop over the next<br />

three years <strong>and</strong> will lead to MD Theses in each case.<br />

Dr Hugh Mulcahy received a three year grant from Cancer<br />

Research Irel<strong>and</strong> to investigate "Protease profiling in early<br />

colorectal cancer: Its role in anatomical <strong>and</strong> clinical progression".<br />

Mr Shane Sullivan <strong>and</strong> Ms Julie Gorman have been appointed as<br />

medical scientists to work on this programme.<br />

Ronan Ryan continues his work identifying immunohistochemical<br />

markers associated with response to chemoradiotherapy<br />

in rectal cancer, while Anne Marie Lennon,Eddie Fox<br />

<strong>and</strong> Denise Keegan are studying the genetic make up of<br />

patients with familial Colorectal Cancer.<br />

The unit presented work at a number of National <strong>and</strong><br />

International meetings including the AGA,USCAP <strong>and</strong> the ISG.<br />

Education<br />

The Tenth International Colorectal Meeting was held in the<br />

Education <strong>and</strong> Research Centre in September. Professor Bruce<br />

Minsky from New York delivered the <strong>St</strong>ate of the Art Lecture<br />

Grace McEvoy again organised a very successful Annual <strong>St</strong>udy<br />

Day for nurses in April.<br />

The centre continued with its policy of giving lectures to the<br />

general public throughout the year.<br />

Hugh Mulcahy continued with his involvement in the Beaumont<br />

Hospital Tips <strong>and</strong> Tricks Courses held in March <strong>and</strong> September.<br />
