The role of the <strong>ARNP</strong> at Carena is an instrumental position supporting the organizational priorities and goals associated<br />

with delivering cost‐effective, timely and high quality health care services. <strong>ARNP</strong>s are responsible for providing medical<br />

care within the scope of family medicine, healthcare education and care assessment and coordination.<br />

The <strong>ARNP</strong> is responsible for ensuring that the medical care provided to Carena patients is consistent with Carena’s<br />

standards for quality care, and that the appropriate level of care is delivered in a timely manner.<br />

<strong>ARNP</strong>s provide expertise and medical services in the following areas:<br />

Patient assessment and evaluation scoring via a proprietary technology‐based tool<br />

Delivery of high quality care via one of three modalities: telephone, web cam or house call<br />

High quality patient encounter documentation<br />

Primary Responsibilities & Duties<br />

Participate in the Medical Group’s obligation to provide 24/7 medical care and services consistent with the<br />

Group’s obligation to Carena to cover its patient population.<br />

Responsibilities during shift are as follows:<br />

o Treat the acute medical need using the following modalities; phone care, web camera, or house call<br />

o Provide medical care in a competent and efficient manner consistent with generally accepted medical<br />

practices and standards prevailing in the professional community<br />

o Provide education and care coordination with the following objectives:<br />

• Use open‐ended questioning approach to obtain a complete medical history<br />

• Identify gaps that exist in a patient’s care and assist patient in developing a plan to address<br />

these areas<br />

• Teach patients about key risk factors and pathophysiology related to prior medical history<br />

and any ongoing underlying conditions and/or, teach specified US Preventive Task Force<br />

during visits<br />

• Coordinate patient referrals to business partner or client‐specific support programs, as<br />

appropriate (i.e., EAP, condition management, smoking cessation, weight management, etc.)<br />

• Coordinate care with patient’s primary physician and other providers as indicated<br />

Ensure processes and protocols are followed in order to provide efficient health care services to patients.<br />

Collaborate with members of the provider group in ensuring a high quality service experience, and that health<br />

care delivery standards are met.<br />

Assure delivery of clinical services in a way that meets and exceeds quality standards, while providing an<br />

excellent patient experience.<br />

Participate and comply with Quality Assurance activities<br />

Participate in weekly provider meetings<br />

Provider must be Board Eligible / Board Certified and maintain valid license to practice medicine and valid DEA<br />

certification without restrictions.<br />

Maintain compliance with all CME requirements consistent with the standards set forth by the AAFP, the AMA<br />

or as otherwise imposed by state law<br />

Maintain eligibility to receive reimbursement from public and private health care programs, payors and<br />

insurance companies as required by Carena and Medical Group.<br />

Maintain a valid driver’s license, a fully functional car and car insurance with coverage limits at levels required<br />

by Carena.<br />

Build and maintain cross‐functional partnerships and working relationships.<br />

Effectively manage the clinical and associated operations responsibilities of a 24/7 service delivery model.<br />

Maintain performance standards, service excellence, and operational efficiency.<br />

Ensure adoption, utilization and continued utilization of best practice provider service standards and<br />

methodologies.<br />

BA/BS or equivalent degree in life sciences field (i.e. nursing, health care administration or other health<br />

profession).<br />

Two years of work experience in a clinical setting preferred.<br />

Prior experience working in a fast paced, high growth clinical setting, preferred.<br />

Experience working in a variety of the clinical or healthcare delivery environments, desired.<br />

Demonstrated effective interpersonal skills including composure, diplomacy, and the ability to interact with a<br />

diverse patient population.<br />

Excellent communication, interpersonal, and writing skills.<br />

Ability to work independently, and demonstrate good personal judgment.<br />

Practical solution orientation.<br />

Interest in, and aptitude and/or experience with the use of emerging technology in the delivery and the<br />

documentation of health care.<br />

Comfortable with shifting priorities and frequent changes that a small company in a burgeoning business often<br />

experience.<br />

Demonstrated ability to handle tasks efficiently under pressure when circumstances dictate a sense of urgency.<br />

Flexibility in meeting workload fluctuations and assisting with a variety of requests and tasks as they arise.<br />
