Make your immunization appointment<br />
Complete these required forms:<br />
Consent to Release Records form<br />
UW Medicine Privacy Agreement<br />
Photo ID form<br />
Blood Borne Pathogen Acknowledgement form<br />
Complete HIPAA training online<br />
Print a copy <strong>of</strong> your HIPAA completion confirmation page<br />
Complete OSHA Hazards Management training online<br />
Print a copy <strong>of</strong> your hazmat training completion confirmation page<br />
Subscribe to the nursgrad listserv<br />
Get your UW ID Card (Husky Card)<br />
Familiarize yourself with the following websites:<br />

You will need to obtain a Health Sciences photo ID badge. (this is different than your Husky Card). Print<br />
and complete the linked form. The photo lab (room T-271 <strong>of</strong> the Health Sciences Building) will have<br />
open hours during one <strong>of</strong> the following times:<br />
Monday, September 21, 2009, 10:20-noon and 2:00-4:00pm – Incoming students with last names ending<br />
in A through L only.<br />
Tuesday, September 22, 2009, 10:20-noon and 2:00-4:00pm – Incoming students with last names ending<br />
in M through Z only.<br />
Your photo badge will cost approximately $35 for an ID badge with an after-hours building access card,<br />
or $30 for an ID badge only.<br />

Download, print, sign, and return to Academic Services (room T-310 in the Health Sciences building) on<br />
the afternoon <strong>of</strong> the first day <strong>of</strong> <strong>Orientation</strong> (September 22).
