APPLICATION GUIDELINES<br />

<strong>General</strong> <strong>Guidelines</strong> – Workshops/ Courses<br />

1. Application Procedure<br />

Interested participants should contact PGAHI to apply for the workshop or course before the<br />

stipulated closing date for registration or fax PGAHI the registration form with details duly<br />

completed. An email confirmation of receipt of registration form will be sent. Participants are<br />

advised that registration is only confirmed upon receipt of full payment.<br />

2. Training dates/ Period<br />

The training dates may change without prior notice, subject to the availability of the training<br />

places and responses from participants.<br />

3. Training fee<br />

PGAHI reserves the right to determine the training fee, which are subjected to change without<br />

prior notice and is inclusive of the prevailing Goods & Services Tax (GST).<br />

4. Payment<br />

Full payment of the training fee should be made at least 2 weeks prior to the commencement of<br />

training.<br />

5. Cancellation Policy<br />

Any cancellation or replacement must be conveyed to PGAHI in writing. A cancellation fee of<br />

50% of registration fee will be charged if the cancellation is received before closing date. There<br />

will be no refund of registration fee for cancellation on or after closing date.<br />

PGAHI reserves the right to cancel the workshop and fully refund the amount of registration fee<br />

paid to the participants should unforeseen circumstances necessitate it.<br />

6. Additional expenses<br />

The participants shall bear any incidental charges incurred such as those for meals, transport,<br />

parking fees, accommodation and laundry.<br />

7. Certification<br />

Upon successful completion of the training, all participants will receive a certificate of attendance<br />

from PGAHI.


Clinical Attachment <strong>Guidelines</strong><br />

1. Basic Criteria<br />

i. A competency level in English language, both written and spoken as the medium of<br />

instruction is English.<br />

ii.<br />

Applicant must be willing and able to furnish all the requested documents from<br />

PGAHI to his / her best ability.<br />

iii. Applicant should note that the training programme does not imply recognition or<br />

acceptance for registration with local professional association.<br />

iv. Applicant must return to his / her country upon completion of training.<br />

2. Application Procedure<br />

Application for clinical attachment training at SGH must be made through the applicant (selffunded)<br />

or applicant’s sponsoring institution (funded by institution).<br />

Interested applicants should send an email request to PGAHI at least 6 months in advance of their<br />

proposed date / month of training. The email request should be accompanied by the following<br />

set of documents stated below:<br />

i. Applicant’s Curriculum Vitae<br />

ii.<br />

Certified true copies of applicant’s relevant educational certificates and transcripts.<br />

For the above documents which are not in English, an official English translation is<br />

required. Certificates in original languages must also be submitted<br />

iii. Testimonies of clinical experience<br />

iv. A completed SGH application form for clinical attachment, with clear learning<br />

objectives stated<br />

v. Name of department in SGH applicant would like to be attached to<br />

vi.<br />

State type of clinical attachment training: Hands-on or Observation<br />

vii. Proposed commencement date / month and duration of attachment<br />

Applicant should note that an application for clinical attachment training is subjected to the<br />

respective department’s approval and availability of places.<br />

If applicant is found unsuitable for training, there will be an email notification.


3. Acceptance<br />

<strong>Guidelines</strong><br />

If applicant is found eligible for training, there will be an email notification from PGAHI. The<br />

acceptance is subjected to the following conditions being fulfilled by the applicant prior to the<br />

commencement date of training:<br />

i. Joint agreement with SGH on the commencement date and duration of training<br />

ii.<br />

Approval of the Training Employment Pass (TEP) from the Ministry of Manpower and<br />

Immigration Department, <strong>Singapore</strong><br />

Prior to the approval, a completed Training Employment Pass (TEP) form must be submitted to<br />

PGAHI. <strong>Singapore</strong>ans / PRs are not required to complete the TEP form.<br />

iii. Submission of the following documents within 3 weeks of email notification<br />

· Hardcopies of the documents listed under item 2 “Application Procedure”<br />

· A clear copy of the personal particulars page on passport / travel document<br />

· A recent passport-sized photograph of applicant against a white background, taken<br />

within the last 3 months<br />

· A copy of Hepatitis B, Hepatitis C and HIV screening results. The results must show<br />

clearly that the applicant has passed the screening.<br />

· A Basic Cardiac Life Support (BCLS) Certificate, valid for the time that applicant is<br />

attached to SGH.<br />

Applicant is advised to attend the BCLS training in his / her home country. Applicant should note<br />

that BCLS training is available at SGH. The cost of the BCLS training is subjected to the prevailing<br />

cost and taxes in effect at the time of training and borne separately by the applicant.<br />

iv. Applicant must arrange for his / her own professional and personal medical /<br />

hospitalization insurance to cover the full training period.<br />

v. Applicant applying for hands-on training is also expected to have professional<br />

malpractice insurance coverage in <strong>Singapore</strong>.<br />

vi. If required, pharmacists on hands-on training must have temporary professional<br />

registration with the <strong>Singapore</strong> Pharmacy Board (SPB) for the duration of training.<br />

The cost of registration and the processing fee will be borne separately by the<br />

applicant.<br />

vii. Where applicable, the applicant may be required to pass a medical examination at<br />

the SGH Staff Clinic. The applicant will be responsible for the expenses incurred.<br />

viii. Upon the email confirmation of acceptance by SGH, a nonrefundable administrative<br />

fee and training fee per week of training per applicant will be levied.<br />

4. Processing Time<br />

It takes approximately 6 months lead time to process a successful application, which includes<br />

the necessary applications to the Ministry of Manpower, <strong>Singapore</strong> Immigration Authority, and<br />

<strong>Singapore</strong> Pharmacy Board etc.


5. Clinical Attachment Fees per applicant<br />

(i) Training fees<br />

SGD$600 per week of training<br />

(ii) Non-refundable administrative fee<br />

SGD$200<br />

(iii) TEP processing fee<br />

SGD$37.38<br />

(iv) Visa processing fee (If applicable)<br />

SGD$28.04<br />

The above fees stated are subjected to the prevailing taxes and may change without prior notice.<br />

The applicant is advised to always check with PGAHI on the latest fees.<br />

6. Additional Estimated Expenses<br />

(i) Accommodation rental per applicant<br />

SGD$500 - $1800 per month<br />

(ii) Food expenditure<br />

SGD$450 per month<br />

(iii) Public transportation cost<br />

(iv) Medical Examination<br />

(It will be applicable if applicant fail to submit results prior<br />

to the commencement of training)<br />

(v) BCLS Training<br />

(It will be applicable if applicant fail to submit proof of<br />

training prior to the commencement of training)<br />

SGD$70 - $100<br />

SGD$120<br />

If applicable, these expenses will be borne separately by the applicant. The list of items stated<br />

above is non-exhaustive and only outlines the basic living expenses in <strong>Singapore</strong>.<br />

7. Payment<br />

Full payment of the fee must be made prior to the commencement of the clinical attachment<br />

training, only after the confirmation of acceptance by SGH.<br />

Payment of fee is in <strong>Singapore</strong> Dollars. Payment in cheque should be crossed and made payable<br />

to “<strong>Singapore</strong> <strong>General</strong> <strong>Hospital</strong> Pte Ltd” to the following address:<br />

There will be no refund of fee should there be an early cessation of the training period by the<br />

applicant. However, should there be any extension of training period, fees that are necessary to<br />

facilitate the extension would be charged accordingly.


8. Accommodation<br />

<strong>Guidelines</strong><br />

Applicant is responsible for his / her housing arrangements while in <strong>Singapore</strong>. PGAHI can assist in<br />

the recommendation of accommodation for overseas applicants. However, applicants will need to<br />

do the booking themselves. Applicants are advised to exercise their own due diligence in selecting<br />

their accommodation.<br />

9. Cancellation Fee<br />

All requests for cancelation or early termination of training must be made in writing. PGAHI<br />

reserves the right to levy a cancellation fee.<br />

10. Responsibilities of the Successful Applicant<br />

i. The trainee is expected to involve himself / herself in the daily routine of the<br />

department.<br />

ii.<br />

The trainee will be under supervision and assessments during the period of his / her<br />

training as required by training programme.<br />

iii. Temporary leave of absence needs approval from the Head of Department. If for<br />

some reasons, the trainee wishes to change the placement or leave prior to<br />

completion of the training programme, permission must be sought from the Head<br />

of Department.<br />

iv. The training programme can only maintain its value or improve, if there is feedback<br />

from the trainee. Following the completion of the training, the trainee will submit<br />

an evaluation form to SGH-PGAHI.<br />

v. SGH will require the sponsoring institution / individual to sign a letter of indemnity<br />

and the trainee to sign a letter of indemnity and non-disclosure and security<br />

awareness agreement.<br />

vi. The trainee is expected to attend all classes assigned by the department or<br />

mandated by SGH such as Infection Control Training etc.<br />

vii. Expenses such as those incurred for training materials, meals, laundry and transport<br />

will be borne by the trainee.
