adults and children (Griffiths 2010). The analgesic effect of <strong>the</strong><br />

TAPB may partially depend on <strong>the</strong> rise in serum concentration of<br />

<strong>the</strong> local anes<strong>the</strong>tic (Kato 2009).<br />

Blood Clearance<br />

In normal healthy persons, <strong>the</strong> amide local anes<strong>the</strong>tics are<br />

bound to plasma α -1-acid-glycoprotein that effectively prevents<br />

<strong>the</strong> presence of high concentrations of unbound and active local<br />

anes<strong>the</strong>tic. Surgery fur<strong>the</strong>r stimulates <strong>the</strong> syn<strong>the</strong>sis of α-1-acid-<br />

glycoprotein from <strong>the</strong> liver, reducing <strong>the</strong> risk of toxicity<br />

(Aronsen 1972, Pettersson 1998).<br />

The clearance of local anes<strong>the</strong>tics is dependent on <strong>the</strong> renal<br />

and hepatic flow and cardiac function. In advanced heart, kidney<br />

and liver failure and <strong>the</strong>rapy with cytochrome isoenzyme<br />

inhibitors like antimycotics, <strong>the</strong> dose of <strong>the</strong> local anes<strong>the</strong>tic<br />

should be reduced by 10 to 50% (Rosenberg 2004).<br />

Age related changes in blood flow and organ function may<br />

increase <strong>the</strong> nerve sensitivity to a local anes<strong>the</strong>tic block, and a<br />

smaller dose is needed to achieve <strong>the</strong> same effect. Local<br />

anes<strong>the</strong>tic doses need to be reduced by up to 20% in <strong>the</strong> elderly<br />

(Rosenberg 2004).<br />

The late stage of pregnancy is characterized by a<br />

physiologically enhanced sensitivity of nerves to local<br />

anes<strong>the</strong>tics. <strong>Blocks</strong> should be per<strong>for</strong>med with <strong>the</strong> lowest possible<br />

doses <strong>for</strong> short periods aiming to reduce <strong>the</strong> need <strong>for</strong> o<strong>the</strong>r<br />

analgesics (Rosenberg 2004).<br />

Pediatric Considerations<br />

Neonates and children up to 4 months of age have low plasma<br />

concentrations of α -1-acid-glycoprotein and thus a greater<br />

amount of free drug in <strong>the</strong> blood (McNamara 2002). A more<br />

conservative dose should be used when per<strong>for</strong>ming an<br />

abdominal block in infants and neonates (< 15 kg) because a
