Lower <strong>Abdominal</strong> Surgery<br />
Lower abdominal surgery includes varicocelectomy, appendicectomy,<br />
open prostatectomy, lumbectomy and intra-aortic<br />
procedures with femoral artery cannulation.<br />
Surgical reports on awake varicocelectomy show <strong>the</strong> efficacy<br />
of local anes<strong>the</strong>tic infiltration beneath <strong>the</strong> aponeurosis of <strong>the</strong><br />
EOM into <strong>the</strong> inguinal canal to block <strong>the</strong> ilioinguinal and<br />
genitofemoral nerves (Hsu 2005). Recently, an effective<br />
ultrasound-guided spermatic cord block was reported (Wipfli<br />
2011).<br />
In <strong>the</strong> only randomized study in adults undergoing<br />
varicocelectomy under general anes<strong>the</strong>sia and an IIB be<strong>for</strong>e<br />
surgery, patients experienced significantly reduced<br />
postoperative pain scores at rest and during mobilization, less<br />
analgesic consumption, less nausea and vomiting and were all<br />
discharged at 6 hours (Yazigi 2002).<br />
The IIB and <strong>the</strong> TAPB have also been evaluated in <strong>the</strong><br />
per<strong>for</strong>mance of appendicectomy. The IIB per<strong>for</strong>med be<strong>for</strong>e<br />
surgery in children undergoing appendicectomy showed better
