<strong>Ultrasound</strong>-guided Iliohypogastric and<br />

Ilioinguinal Nerve Block<br />

The transducer is placed over <strong>the</strong> mid-axillary line and above<br />

<strong>the</strong> iliac crest (Figure 4.2). The best image is tracked by moving<br />

<strong>the</strong> transducer along <strong>the</strong> course of <strong>the</strong> iliac crest in <strong>the</strong> direction<br />

of <strong>the</strong> ASIS (Figure 4.3). When positioning <strong>the</strong> transducer, <strong>the</strong><br />

three muscular layers of <strong>the</strong> abdominal wall will be seen on <strong>the</strong><br />

screen. The iliac bone will be seen at one side of <strong>the</strong> screen as<br />

black. On <strong>the</strong> o<strong>the</strong>r side of <strong>the</strong> screen, deeper, <strong>the</strong> abdominal<br />

cavity and eventually peritoneum or <strong>the</strong> bowel may be seen.<br />

The three abdominal muscles, <strong>the</strong> EOM, <strong>the</strong> IOM and <strong>the</strong> TAM,<br />

are seen as hypo-echoic longitudinal bands (Figure 4.1). The IOM<br />

is <strong>the</strong> thickest and <strong>the</strong> TAM is <strong>the</strong> deepest. The muscular fascias<br />

between <strong>the</strong>m are seen as hyper-echoic and hyper-lucent. Along<br />

<strong>the</strong> fascia between <strong>the</strong> IOM and <strong>the</strong> TAM, two oval structures
