Richard Kolesar provided the raw footage for the laryngoscopy<br />

Rob Whyte allowed the use of his animated slides illustrating the<br />

concepts of fluid compartments. <strong>The</strong> image of the “tank” of water<br />

was first developed by Dr. Kinsey Smith, who kindly allowed the<br />

use of that property for this book.<br />

Joan and Nicholas Scott (wife and son of D. Bruce Scott) generously<br />

allowed the use of material from “Introduction to <strong>Regional</strong><br />

Anaesthesia” by D. Bruce Scott (1989).<br />
