DIPHENHYDRAMINE<br />

Class<br />

Antihistamine, antiemetic. Used in the treatment of pruritis,<br />

allergic reactions and drug-induced extrapyramidal<br />

reactions.<br />

Mechanism of Action<br />

Diphenhydramine is a competitive inhibitor at the histamine<br />

H1 receptor. <strong>The</strong> antiemetic effects is related to<br />

central anticholinergic effect as well as histamine antagonism<br />

in the vestibular system in the brain.<br />

Misc.<br />

May cause dry mouth, blurred vision, difficult urination;<br />

more rarely causes acute glaucoma or worsening<br />

of asthma, and GI or GU obstruction. <strong>The</strong>se side effects<br />

reflect its anticholinergic activity, which is additive<br />

with other anticholinergics. and monoamine oxidase<br />

inhibitors (MAOI).<br />

Dose<br />

Adults: 25-50 mg PO q6-8 hours; 10-50 mg IV/IM q 6<br />

hours; maximum daily dose 400 mg.<br />
