LIDOCAINE<br />

Class<br />

Local anesthetic. Used in infiltration anesthesia and regional<br />

anesthesia (e.g. intravenous regional anesthesia).<br />

Lidocaine is still used for epidural anesthesia, especially<br />

for Caesarian section. Lidocaine is rarely used in spinal<br />

anesthesia due to associated nerve irritation. Lidocaine is<br />

occasionally used in the treatment of ventricular arrhythmias.<br />

Mechanism of Action<br />

Sodium channel blocker<br />

Dose<br />

Anesthetic:<br />

Maximum 4 mg/kg without epinephrine<br />

Maximum 7 mg/kg with epinephrine<br />

Anti-arhythmic:<br />

1 mg/kg IV bolus followed by 0.5 mg/kg q 2-5minutes<br />

to maximum 3 mg/kg/hr<br />

By infusion (of 0.1% solution): 1-4 mg/min (20-50 ug/<br />

kg/min)<br />

Onset<br />

IV: 45-90 seconds<br />

Infiltration: 0.5-1 minute<br />

Epidural: 5-15 minutes<br />
