<strong>Instructions</strong> for Completing Part I of EPA Form R<br />

The North American Industry Classification System (NAICS)<br />

is a new economic classification system that will replace the<br />

1987 SIC code system. EPA will address the SIC code change,<br />

as it relates to EPCRA, in an upcoming Federal Register notice.<br />

This upcoming change does NOT affect the 2003 EPCRA<br />

section 313 reporting.<br />

4.6 Latitude <strong>and</strong> Longitude<br />

Enter the latitude <strong>and</strong> longitude coordinates of your facility.<br />

Sources of these data include EPA permits (e.g., NPDES<br />

permits), county property records, facility blueprints, <strong>and</strong> site<br />

plans. You can determine the latitude <strong>and</strong> longitude of your<br />

facility using a siting tool found on the TRI Home page or in<br />

the TRI-ME software. For information on the siting tool <strong>and</strong><br />

instructions on how to determine these coordinates see<br />

Appendix E. Enter only numerical data. Do not preface<br />

numbers with letters such as N or W to denote the hemisphere.<br />

Latitude <strong>and</strong> longitude coordinates of your facility are very<br />

important for pinpointing the location of reporting facilities <strong>and</strong><br />

are required elements on the Form R. EPA encourages<br />

facilities to make the best possible measurements when<br />

determining latitude <strong>and</strong> longitude. Please check to make sure<br />

the latitude <strong>and</strong> longitude coordinates of your facility are<br />

correct. For the continental United States readings should be<br />

within 24E23'58" <strong>and</strong> 49E22'16" latitude, <strong>and</strong> 66E53'06" <strong>and</strong><br />

124E50'55" longitude. For Alaska readings should be within<br />

51E10'30" <strong>and</strong> 71E26'04" latitude, <strong>and</strong> 129E59'29" <strong>and</strong><br />

187E39'08" longitude. For Hawaii readings should be within<br />

18E51'56" <strong>and</strong> 28E30'59" latitude, <strong>and</strong> 154E45'21" <strong>and</strong><br />

178E26'25" longitude. As with any other data field, missing,<br />

suspect, or incorrect data may generate an error notice in the<br />

Facility Data Profile to be issued to the facility. (See<br />

Appendix C)<br />

4.7 Dun & Bradstreet Number(s)<br />

Enter the nine-digit number assigned by Dun & Bradstreet (D<br />

& B) for your facility or each establishment within your<br />

facility. These numbers code the facility for financial purposes.<br />

This number may be available from your facility*s treasurer or<br />

financial officer. You can also obtain the numbers from your<br />

local Dun & Bradstreet office (check the telephone book White<br />

Pages). If a facility does not subscribe to the D & B service, a<br />

number can be obtained, toll free at 800 234-3867 (8:00 AM to<br />

6:00 PM, Local Time) or on the Web at . If<br />

none of your establishments has been assigned a D & B<br />

number, you should enter NA in box (a). If only some of your<br />

establishments have been assigned D & B numbers, enter those<br />

numbers in Part I, section 4.7.<br />

4.8 EPA Identification Number(s)<br />

The EPA Identification Number is a 12-character number<br />

assigned to facilities covered by hazardous waste regulations<br />

under the Resource Conservation <strong>and</strong> Recovery Act (RCRA).<br />

Facilities not covered by RCRA Subtitle C are not likely to have<br />

an assigned identification number. If your facility is not<br />

required to have an identification number, you should enter NA<br />

in box (a). If your facility has been assigned EPA Identification<br />

Numbers, you must enter those numbers in the spaces provided<br />

in Section 4.8.<br />

4.9 NPDES Permit Number(s)<br />

Enter the numbers of any permits your facility holds under the<br />

National Pollutant Discharge Elimination System (NPDES) even<br />

if the permit(s) do not pertain to the EPCRA section 313<br />

chemical being reported. This nine-character permit number is<br />

assigned to your facility by EPA or the state under the authority<br />

of the Clean Water Act. If your facility does not have a permit,<br />

you should enter NA in Section 4.9a.<br />

4.10 Underground Injection<br />

Well Code (UIC) Identification Number(s)<br />

If your facility has a permit to inject a waste that contains or<br />

contained the EPCRA section 313 chemical into Class 1 deep<br />

wells, enter the 12-digit Underground Injection Well Code<br />

(UIC) identification number assigned by EPA or by the state<br />

under the authority of the Safe Drinking Water Act. If your<br />

facility does not hold such a permit(s), you should enter NA in<br />

Section 4.10a. You are required to provide the UIC number for<br />

wells that receive the EPCRA section 313 chemical being<br />

reported in the current reporting year.<br />

Section 5. Parent Company Information<br />

You must provide information on your parent company. For<br />

purposes of Form R, a parent company is defined as the highest<br />

level company, located in the United States, that directly owns<br />

at least 50% of the voting stock of your company. If your<br />

facility is owned by a foreign entity, enter NA in this space.<br />

Corporate names should be treated as parent company names for<br />

companies with multiple facility sites. For example, the<br />

Bestchem Corporation is not owned or controlled by any other<br />

corporation but has sites throughout the country whose names<br />

begin with Bestchem. In this case, Bestchem Corporation<br />

should be listed as the parent company. Note that a facility that<br />

is a 50:50 joint venture is its own parent company. When a<br />

facility is owned by more than one company <strong>and</strong> there is no<br />

parent company for the entire facility (meaning that none of the<br />

facility owners directly owns at least 50 percent of the voting<br />

stock of the facility at issue), the facility should provide the<br />

name of the parent company of either the facility operator or the<br />

<strong>Toxic</strong>s <strong>Release</strong> <strong>Inventory</strong> <strong>Reporting</strong> <strong>Forms</strong> <strong>and</strong> <strong>Instructions</strong> 33
