<strong>Instructions</strong> for Completing Part I of EPA Form A<br />

If you have submitted a Form A or Form R for previous<br />

reporting years, a TRI Facility Identification Number has<br />

been assigned to your facility. If you know your TRI Facility<br />

Identification Number, complete Section 4. If you do not<br />

know your TRI Facility Identification Number, contact the<br />

EPCRA Call Center (see page 7). If your facility has moved,<br />

do not enter your TRI facility identification number, you<br />

should enter “New Facility.”<br />

The TRI Facility Identification Number is established by the<br />

first Form R submitted by a facility at a particular location.<br />

This identification number is retained by the facility even if<br />

the facility changes name, ownership, production processes,<br />

SIC codes, etc. This identification number will stay with this<br />

location. If a new facility moves to this location it should use<br />

this TRI Facility Identification Number. Establishments of a<br />

facility that report separately should use the TRI Facility<br />

Identification Number of the facility.<br />

You should enter “New Facility” in the space for the TRI<br />

Facility Identification number if this is your first submission.<br />

4.2 Federal Facility Designation<br />

Executive Order 13148 directs federal facilities to comply<br />

with Right-To-Know Laws <strong>and</strong> Pollution Prevention<br />

Requirements. Please indicate in 4.2.C. if the reporting<br />

facility is a federal facility or in 4.2.D if the submitter is a<br />

contractor at a federal facility (GOCO). If the reporting<br />

facility is not a federal facility, you should leave this space<br />

blank. Form R allows a facility to report multiple<br />

submissions for the same chemical if the facility is composed<br />

of several distinct establishments. This data element provides<br />

the option of reporting full or partial facility information on<br />

Form R, however, this is not applicable for those facilities<br />

taking advantage of the Alternate Threshold <strong>and</strong> Form A.<br />

An explanation of this is provided in Section D.<br />

4.3 Technical Contact<br />

Enter the name <strong>and</strong> telephone number (including area code)<br />

of a technical representative whom EPA or state officials may<br />

contact for clarification of the information reported on Form<br />

A. You should also enter an email address for this person. If<br />

the technical contact does not have an email address you<br />

should enter NA. This contact person does not have to be the<br />

same person who prepares the report or signs the Form A <strong>and</strong><br />

does not necessarily need to be someone at the location of the<br />

reporting facility. However, this person should be familiar<br />

with the details of the report so that he or she can answer<br />

questions about the information provided.<br />

4.4 Intentionally Left Blank<br />

4.5 St<strong>and</strong>ard Industrial Classification (SIC) Code<br />

Enter the appropriate four-digit St<strong>and</strong>ard Industrial<br />

Classification (SIC) Code that is the primary SIC Code for<br />

your facility in Section 4.5(a). Enter any other applicable<br />

SIC Codes for your facility in 4.5 (b)–(f). Table I lists the<br />

SIC codes within 10 (except 1011, 1081, <strong>and</strong> 1094), 12<br />

(except 1241), 20–39, 4911 (limited to facilities that combust<br />

coal <strong>and</strong>/or oil for the purpose of generating electricity for<br />

distribution in commerce), 4931 (limited to facilities that<br />

combust coal <strong>and</strong>/or oil for the purpose of generating<br />

electricity for distribution in commerce), 4939 (limited to<br />

facilities that combust coal <strong>and</strong>/or oil for the purpose of<br />

generating electricity for distribution in commerce), 4953<br />

(limited to facilities regulated under the RCRA Subtitle C, 42<br />

U.S.C. section 6921 et seq.), 5169, 5171, <strong>and</strong> 7389 (limited to<br />

facilities primarily engaged in solvents recovery services on a<br />

contract or fee basis). If the report covers more than one<br />

establishment, enter the primary four-digit SIC code for each<br />

establishment starting with the primary SIC code for the<br />

entire facility. You are required to enter SIC codes only for<br />

those establishments within the facilities that fall within SIC<br />

codes 10 (except 1011, 1081, <strong>and</strong> 1094), 12 (except 1241),<br />

20–39, 4911 (limited to facilities that combust coal <strong>and</strong>/or oil<br />

for the purpose of generating electricity for distribution in<br />

commerce), 4931 (limited to facilities that combust coal<br />

<strong>and</strong>/or oil for the purpose of generating electricity for<br />

distribution in commerce), 4939 (limited to facilities that<br />

combust coal <strong>and</strong>/or oil for the purpose of generating<br />

electricity for distribution in commerce), 4953 (limited to<br />

facilities regulated under the RCRA Subtitle C, 42 U.S.C.<br />

section 6921 et seq.), 5169, 5171, <strong>and</strong> 7389 (limited to<br />

facilities primarily engaged in solvents recovery services on a<br />

contract or fee basis). If you do not know your SIC code,<br />

consult the 1987 SIC Manual (see section B.2 of these<br />

instruction for ordering information).<br />

The North American Industry Classification System<br />

(NAICS), is a new economic classification system that will<br />

replace the 1987 SIC code system. EPA will address the SIC<br />

code change, as it relates to EPCRA, in an upcoming Federal<br />

Register notice. This upcoming change does NOT affect the<br />

2003 EPCRA section 313 reporting.<br />

4.6 Latitude <strong>and</strong> Longitude<br />

Enter the latitude <strong>and</strong> longitude coordinates of your facility.<br />

Sources of these data include EPA permits (e.g., NPDES<br />

permits), county property records, facility blueprints, <strong>and</strong> site<br />

plans. You can determine the latitude <strong>and</strong> longitude of your<br />

facility using a siting tool found on the TRI Home page. For<br />

information on the siting tool <strong>and</strong> instructions on how to<br />

determine these coordinates see Appendix E. Enter only<br />

numerical data. Do not preface numbers with letters such as<br />

N or W to denote the hemisphere.<br />
