counseling and learning ways to live a healthier<br />

life. Together with medical and surgical<br />

treatments, cardiac rehab can help you recover<br />

faster and gain strength, gradually increase<br />

your exercise tolerance, reduce risk factors to<br />

help prevent future heart problems and provide<br />

valuable support.<br />

<strong>Cardiac</strong> <strong>Rehab</strong> helps you to recognize and<br />

change unhealthy habits and establish new,<br />

healthier ones. Your rehab may last from<br />

4-12 weeks, and it is important to complete<br />

the recommended plan.<br />

You can benefit from cardiac rehab if you have<br />

had a heart attack or heart failure, have angina<br />

or have had angioplasty, coronary artery bypass<br />

or valve surgery.<br />

Almost everyone with heart disease can benefit<br />

from some type of cardiac rehab. No one is too<br />

old or too young, and women and men benefit<br />

equally.<br />

How does<br />

cardiac rehab work?<br />

Exercise Training<br />

This helps you learn how to exercise safely,<br />

strengthen your muscles and improve your<br />

stamina. Your exercise will be based on your<br />

individual ability, needs and interests.<br />

Language assistance services are available free of charge for<br />

anyone who has a need for an interpreter.<br />

Education, Counseling and Training<br />

These will help you understand your heart<br />

condition and find ways to reduce your risk of<br />

future heart problems. The cardiac rehab team<br />

will help you learn how to cope with the stress<br />

of adjusting to a new lifestyle and to deal with<br />

your fear about the future.<br />

Recovering from Heart Problems<br />

through <strong>Cardiac</strong> <strong>Rehab</strong>ilitation


<strong>Cardiac</strong> <strong>Rehab</strong>ilitation<br />

Inpatient Program – Phase I<br />

Phase I <strong>Cardiac</strong> <strong>Rehab</strong>ilitation includes<br />

education and supervised activity for hospital<br />

patients recovering from a cardiac illness.<br />

Outpatient Program – Phase II<br />

Phase II <strong>Cardiac</strong> <strong>Rehab</strong>ilitation includes ECGmonitored<br />

exercise training, education and<br />

counseling to help the individual understand<br />

their heart condition and find ways to reduce<br />

risk of future heart problems. A physician<br />

referral begins the Phase II rehab process.<br />

Generally classes are three times per week for<br />

about 1 to 1½ hours. Free parking is offered.<br />

Maintenance/Phase III<br />

Candidates for Phase III are individuals with<br />

known coronary disease who are referred by<br />

their physician or who have completed Phase II<br />

<strong>Cardiac</strong> <strong>Rehab</strong>. Individuals have the benefit of<br />

continuing their prescriptive exercise program<br />

in a supervised setting and modifying cardiac<br />

risk factors.<br />

Phase III open sessions are offered at various<br />

times.<br />

Special Programs<br />

Smoking Cessation Program<br />

<strong>Kaleida</strong> <strong>Health</strong>’s <strong>Cardiac</strong> <strong>Rehab</strong> team wants to<br />

help you become smoke-free. The four-session<br />

smoking cessation program includes group<br />

support and motivational tips, healthy lifestyle<br />

changes and using the New York State Quit<br />

Line for support and free nicotine replacement<br />

products. Instruction also includes the use of<br />

nicotine replacement products and information<br />

regarding reimbursement from your insurance<br />

company upon successful completion of the<br />

program.<br />

These programs are conducted three to five<br />

times per year.<br />

Our <strong>Cardiac</strong> <strong>Rehab</strong> Team<br />

Our <strong>Cardiac</strong> <strong>Rehab</strong> staff uses a multidisciplinary<br />

team approach. Members of the team who will<br />

be involved in your care are registered nurses,<br />

physical therapists, exercise physiologists and<br />

registered dietitians. Your physician(s) will<br />

be involved regarding your progress and/or<br />

changes, on a regular basis.<br />

Our professionals provide medical supervision,<br />

education, exercise guidance and nutrition<br />

counseling.
