4. George Malley, Chief Counsel, Langley Research Center, will serve as<br />

counsel to <strong>the</strong> Board.<br />

5. <strong>The</strong> Board will:<br />

a. Review <strong>the</strong> circumstances surround<strong>in</strong>g <strong>the</strong> accident to establish<br />

<strong>the</strong> probable cause or causes of <strong>the</strong> accident, <strong>in</strong>clud<strong>in</strong>g review<br />

of <strong>the</strong> f<strong>in</strong>d<strong>in</strong>gs, corrective action, and recommendations be<strong>in</strong>g<br />

developed by <strong>the</strong> Program Offices, Field Centers, and contractors<br />

<strong>in</strong>volved.<br />

b. Direct such fur<strong>the</strong>r specific <strong>in</strong>vestigations as may be necessary.<br />

c. Report its f<strong>in</strong>d<strong>in</strong>gs relat<strong>in</strong>g to <strong>the</strong> cause of <strong>the</strong> accident to <strong>the</strong><br />

adm<strong>in</strong>istrator as expeditiously as possible and release such<br />

<strong>in</strong>formation through <strong>the</strong> Office of Public Affairs.<br />

d. Consider <strong>the</strong> impact of <strong>the</strong> accident on all Apollo activities<br />

<strong>in</strong>volv<strong>in</strong>g equipment preparation, test<strong>in</strong>g, and flight operations.<br />

e. Consider all o<strong>the</strong>r factors relat<strong>in</strong>g to <strong>the</strong> accident, <strong>in</strong>clud<strong>in</strong>g<br />

design, procedures, organization, and management.<br />

f. Develop recommendations for corrective or o<strong>the</strong>r action based<br />

upon its f<strong>in</strong>d<strong>in</strong>gs and determ<strong>in</strong>ations.<br />

g. Document its f<strong>in</strong>d<strong>in</strong>gs, determ<strong>in</strong>ations, and recommendations<br />

and submit a f<strong>in</strong>al report to <strong>the</strong> Adm<strong>in</strong>istrator which will not be<br />

released without his approval.<br />

6. <strong>The</strong> Board may call upon any element of NASA for support, assistance,<br />

and <strong>in</strong>formation.<br />
