Two Types of Tobacco-Free Credits
If you enroll in Alcoa medical/prescription
drug benefits for 2013, you may be eligible
for one or both of the following credits toward
the cost of those benefits:
Employee Tobacco-Free Credit
If you declare yourself to be tobacco-free, you
are eligible for an $880 annual credit to offset
the cost of your Alcoa-provided medical/
prescription drug benefits. If you don’t enroll
in medical/prescription drug benefits, you are
not eligible for this credit.
Dependent Tobacco-Free Credit
If you declare that all of your covered
dependents are tobacco-free, you are eligible
for another $880 annual credit to offset
the cost of your Alcoa-provided medical/
prescription drug benefits. The credit is the
same regardless of how many dependents
you enroll, as long as they are all tobaccofree.
If you don’t enroll at least one of your
dependents in medical/prescription drug
benefits, you are not eligible for this credit.
However, you are eligible for this credit even
if you are not eligible for the Employee
Tobacco-Free Credit (you use tobacco but
your dependents do not).
The credits are applied to your per-pay cost
for your benefits. For example, if you have
employee-only coverage and are paid monthly,
the cost of your benefits will be reduced by
$73.33 each month if you are tobacco-free.
What Does It Mean To Be “Tobacco-Free”?
• “Tobacco-free” means that you do not use
tobacco in any form — cigarettes, cigars,
pipes, snuff, or chewing tobacco.
• To be considered a non-tobacco user and
eligible for the tobacco-free credit:
––
you must not have used any type of
tobacco product for a period of 30
days before the date you elect medical/
prescription drug coverage (or for a
period of 30 days before the date that
you apply during the year for the credit)
––
you must pledge to remain tobacco-free
Using tobacco on one occasion is considered
using tobacco. If you have done this within
30 days of enrollment, you are not eligible
for the credit.
If you or a dependent use tobacco now and
quit after enrollment, you will be eligible for
the credit if you submit a signed Tobacco
Status Change Form (available online at
MyAlcoaBenefits.com or HRDirect, or by
calling 1-888-ALCOA123 (1-888-252-6212).
You will receive a prorated amount of the
credit. For example, if you quit in March and
send in your form in April, you will receive
eight months’ worth of the credit.
If it is unreasonably difficult (due to a medical
condition) or medically inadvisable for you or
a dependent to stop using tobacco products,
you may still qualify for the tobacco-free credit
by having your physician complete a Tobacco
Use Certification Form, which is available
online at MyAlcoaBenefits.com or HRDirect, or
by calling 1-888-ALCOA123 (1-888-252-6212).
Providing false information related to the
tobacco-free credit could result in disciplinary
action up to and including termination.
More information about the tobacco-free
credit, including free tobacco-cessation
