scheme of LEI-Lembaga Ekolabel Ind<strong>on</strong>esia and FSC-Forest Stewardship Council) and Lead Auditor for FSC<br />

Track of Custody. Other experiences: He is a registered IRCA Lead auditor for Envir<strong>on</strong>mental Management<br />

System and Quality Management System. Since 1999, he has worked for an independent certificati<strong>on</strong> agency<br />

and has c<strong>on</strong>ducted a number of Audits of Envir<strong>on</strong>mental Management System, Quality Management System,<br />

and RSPO in Malaysia and Ind<strong>on</strong>esia. Currently he works General Manager of Management System<br />

Certificati<strong>on</strong>. In Stage-1 activity, he c<strong>on</strong>ducted the assessment <strong>on</strong> legal and envir<strong>on</strong>mental aspects.<br />

2. Yudwi Wisnu Rahmanto (Auditor). Bachelor of Forestry with Silviculture background. He has attended in<br />

pests, and c<strong>on</strong>trol methods that c<strong>on</strong>duct in case of attack. Owls and beneficial plant m<strong>on</strong>itoring is programmed every m<strong>on</strong>th.<br />

that are categorized as World Health Organizati<strong>on</strong> Type 1A or 1B, or are listed by the Stockholm or Rotterdam<br />

Agrochemical used by LMI were not as WHO type 1A or 1B, however the chemicals were registered by agricultural agency, for<br />

Report of Advisory committee of Occupati<strong>on</strong>al Health and Safety at Rantau mill <strong>on</strong> 7 July 2012, which discussed the evaluati<strong>on</strong> of<br />

findings (issues) and their progress of completi<strong>on</strong>. That report focuses <strong>on</strong> safety c<strong>on</strong>diti<strong>on</strong> of each stati<strong>on</strong> (loading ramp, stew,<br />

dings, nut and kernel, press, clarificati<strong>on</strong>, boiler and laboratory).<br />

LMI provides Employee Social Security (Jamsostek) and life insurance to all employees as the insurance for accidents at work.<br />

and mill has determined an assembling point if there is earthquake/fire. Simulati<strong>on</strong> of fire fighting was c<strong>on</strong>ducted <strong>on</strong> July 2012 at<br />

Rantau factory.<br />

Checklists or m<strong>on</strong>itoring of Fire extinguisher standardizati<strong>on</strong> in each estate (betung, matalok, Rantau, sekayu) and mill (Rantau<br />

Factory). Those fire extinguishers are distributed to all locati<strong>on</strong>s that are easily accessible if needed. Locati<strong>on</strong>s of APAR (Fire<br />

extinguisher) placement include guess house, workshop, central warehouse, generator house, house of employee, polyclinics<br />

and elementary school. The company has checked the eligibility and c<strong>on</strong>diti<strong>on</strong> of APAR (Fire extinguisher). Based <strong>on</strong> visit to<br />

Matalok estate, m<strong>on</strong>itoring has been implemented well.<br />

LMI also provides Pers<strong>on</strong>al Protecti<strong>on</strong> Equipments for activities in estate and mill, such as helmets, Apr<strong>on</strong> clothes, gloves,<br />

goggles and masks for spray workers at Matalok estate. First Aid kit also provided by company to workers.<br />

meeting <strong>on</strong> 4 may 2011 at Matalok estate, which was attended by mandors, harvester, spraying workers, and general staff has<br />

Records of work accident regularly m<strong>on</strong>itored every 3 m<strong>on</strong>th. Based <strong>on</strong> work accident report at Rantau estate there were 9 cases<br />

in period of July – September 2012 lead to 48 days lost time accident (LTA) and at Matalok estate work accident report there<br />

were main accident of exposed spines when harvesting. From the accident records LMI has evaluate how to minimize the<br />

accident happens repeated by safety awareness training for harvesters.<br />

the Standard Operating Procedur Manual Policy No 2xx/PTK-LPG/2004 Index Number II/04, <strong>on</strong> 1 December 2004, and HPO<br />

(Head Plantati<strong>on</strong> Operati<strong>on</strong>s) Memorandum No. POD-UM-150/VII/2010 regarding M<strong>on</strong>so<strong>on</strong> Seas<strong>on</strong> Anticipati<strong>on</strong> and Preparati<strong>on</strong><br />

Against Fire Hazard and Draught 2010.<br />

LMI has identified the water sources in company operati<strong>on</strong>al area near the potentially fire, it was for anticipati<strong>on</strong> in case there is<br />

land fire in that area. Field observati<strong>on</strong> presented no evidence of fire usage in the company area.<br />

LMI has the means and infrastructure for fire fighting, which the fire fighting organizati<strong>on</strong> structure for every estate and the<br />

coordinator is Sime<strong>on</strong> Bangun and members. Based <strong>on</strong> field observati<strong>on</strong>s at Rantau estate and Matalok Estate, it was known<br />

that fire land infrastructures and facilities are available and well-functi<strong>on</strong>ed (water machine, hose, nozzle, firefighter uniform,<br />
