AGENDA FOR TRANSITION FROM PRESCHOOL TO KINDERGARTEN MEETING<br />

<strong>IEP</strong> DOCUMENTATION<br />

1) Invitation <strong>to</strong> Conference<br />

A. All observations should be completed prior <strong>to</strong> scheduling the meeting.<br />

B. Parents must be given 10-day notice with a list of all team members that<br />

will be present.<br />

C. Attach a <strong>Transition</strong> <strong>to</strong> <strong>Kindergarten</strong> parent letter <strong>to</strong> the Invitation including a<br />

checklist of what the parent needs <strong>to</strong> bring.<br />

D. Notify all team members with 10-day notice (the parent invitation form can be<br />

copied and sent <strong>to</strong> team members as notification of the meeting).<br />

2) Individual Education Plan (DEC 4) (A psychologist and program specialist must be present if<br />

separate/specialized setting is being considered)<br />

A. Give parent the Handbook of Parent Rights and introduce all team members.<br />

B. It is recommended that annual reviews be completed prior <strong>to</strong> the transition meeting<br />

<strong>to</strong> expedite the actual transition meeting. Review goals; may be updated if needed as<br />

the goals must be appropriate before placement decision is made.<br />

C. Using the Easy<strong>IEP</strong> <strong>Kindergarten</strong> transition process, complete the <strong>IEP</strong>.<br />

(i.) Under Student Profile, review and make changes if necessary <strong>to</strong> Student’s<br />

overall strengths, parent concerns and vision. Address consideration of<br />

transition as the child will be transitioning <strong>from</strong> preschool <strong>to</strong><br />

<strong>Kindergarten</strong>.<br />

(ii.) Under Special Fac<strong>to</strong>rs, review and make changes as appropriate.<br />

(iii.) Least Restrictive Environment I. General Education Program Participation:<br />

Address all general education classes, nonacademic services and<br />

activities and implementation specifications the child will be involved in<br />

and note any supplemental aids/services, modifications and<br />

accommodations or assistive technology needed for <strong>Kindergarten</strong>.<br />

(iv.) Address Section V. Specially Designed Instruction, Related Services, and<br />

Nonacademic Services and Activities, Anticipated Frequency and Location<br />

of Specially Designed Instruction, Anticipated Frequency and Location of<br />

Related Services, Nonacademic Services and Activities and Transportation<br />

as a related service.<br />

(v.) Address Section VI. Continuum of Alternative Educational Placements,<br />

determine and mark the school-age placement according <strong>to</strong> the amount<br />

of time the student is removed <strong>from</strong> nondisabled peers.<br />

0-1 hr. 14 min. a day – Regular setting<br />

1 hr. 15 min. -3 hr. 53 min. a day – Resource setting<br />

3 hr. 54 min. – up a day – Separate setting

Address Section VII. Least Restrictive Environment Justification<br />

Statement – If the child is removed… If this changes, explain WHY the<br />

services cannot be delivered with nondisabled peers with the use of<br />

supplemental aids and services.<br />

Address Section VIII. Progress Toward Annual Goals – must address<br />

if specified otherwise<br />

Team members sign under Section X. Record of <strong>IEP</strong> Team Participation.<br />

3) Prior Written Notice (DEC 5)<br />

A. Address purpose of meeting<br />

B. Address: Educational placement is being changed <strong>from</strong> ________ <strong>to</strong> _________.<br />

(Example: Your child’s alternative placement on the continuum is being changed<br />

<strong>from</strong> PKEC/<strong>Preschool</strong> Early Childhood <strong>to</strong> Regular for <strong>Kindergarten</strong>.<br />

C. Under Other Purpose of <strong>Meeting</strong>, address <strong>Kindergarten</strong> transition, Transportation if<br />

added as a related service (must complete Transportation as a Related Service<br />

Determination form and New Bus Rider Information form), and Extended School Year<br />

(ESY) if considering.<br />

D. Address: Explanation of Action(s) Proposed or Refused – ALL must be documented<br />

and specifically addressed. The least restrictive environment must always be<br />

considered; this is the child’s first placement in a school-age program. Regular and<br />

resource should be considered even if the team decides that the best option <strong>to</strong> meet<br />

the child’s needs is a separate, specialized setting.<br />

E. The receiving school gives the parent a <strong>Kindergarten</strong> registration packet.<br />

4) FINAL STEPS<br />

A. For separate placement, the preschool teacher drafts the “Separate/Specialized<br />

Placement Consideration” form and it is finalized and signed at the transition<br />

meeting.<br />

B. Both the sending and receiving schools complete an Admission/Transfer form and<br />

notifies the NCWISE opera<strong>to</strong>rs of the addition/deletion.<br />

C. The EC folder will be given <strong>to</strong> the receiving SNA at the by the last workday of the<br />

school year and the SNA will sign off that it has been received. For children<br />

determined <strong>to</strong> be eligible for ESY services follow all ESY procedures.
