DOMESTIC MULTIFAMILY ACCOMMODATION<br />

SUBMETERED DECLARATION<br />

Customer has requested a change of rate schedule as provided by Sections D.2.c. and D.2.e. of SCE's Rule 12, from<br />

Schedule to Schedule for service located at<br />

(Building) and identified by SCE as Account Number<br />

(Account). The following declaration is required in order to determine eligibility for service under Southern California<br />

Edison’s Schedule DMS-1, <strong>Domestic</strong> Service <strong>Multifamily</strong> <strong>Accommodation</strong> - <strong>Submetered</strong>.<br />

1. Customer declares one or more of the following conditions applies to Customer’s Account: (Please check box which<br />

applies.)<br />

! Customer is currently served on Schedule DM and has submeters installed for each tenant space.<br />

! Building was originally constructed for a non-residential purpose, was converted to residential use before<br />

December 7, 1981, and has submeters installed for each tenant space.<br />

! Building was originally constructed for a non-residential purpose and was converted to residential use on or<br />

after December 7, 1981 without the need for building permit, and has submeters installed for each tenant<br />

space,<br />

! Other, please specify<br />

2. Number of submeters: Type of space heating: Gas ! Electric !<br />

3. Customer understands that if Customer is found eligible for service under Schedule DMS-1:<br />

a. When service under Schedule DMS-1 commences, tenants shall be billed with separately identifiable charges<br />

for their electric consumption as if served under an applicable SCE residential rate schedule, and Customer<br />

shall reduce the rent charged to tenants to reflect the removal of the energy related charges currently<br />

embedded in the rental charge for the duration of the lease.<br />

b. SCE has the right to inspect the customer's premises and electrical equipment to determine if, in SCE's<br />

opinion, a change in operating/billing conditions has occurred.<br />

c. The effective date of the change to service under Schedule DMS-1 shall be the next regularly-scheduled<br />

billing period following (1) the date of the written request to SCE for the change of rate schedule, (2) the<br />

date, verified by SCE, of the change in operating/billing conditions, or (3) the date this <strong>Declaration</strong> is signed by<br />

Customer, whichever is later.<br />

d. This <strong>Declaration</strong> in no way alters, replaces or supersedes the provisions of Rule 12 or Rule 18.<br />

By signing this <strong>Declaration</strong>, I certify under penalty of perjury under the laws of the State of California that the<br />

information submitted regarding the change in operating/billing conditions for the account specified in this<br />

document is true and accurate.<br />
