• Where airborne exposure monitoring reveals an exposure level routinely above<br />

the action level (or in the absence of an action level, the Permissible Exposure<br />

Limit) for an OSHA regulated substance for which there are exposure monitoring<br />

and medical surveillance requirements. Action level means the airborne<br />

concentration of a specific chemical, identified by OSHA, and calculated as an 8-<br />

hour time weighted average (TWA).<br />

• Whenever an event such as a spill, leak, explosion or other occurrence takes place<br />

and results in the likelihood of a hazardous exposure. In such an event, the<br />

employee shall be provided an opportunity for a medical consultation. The<br />

consultation shall be for the purpose of determining the need for further medical<br />

examination.<br />

XXVII. RECORD KEEPING<br />

<strong>Queensborough</strong> <strong>Community</strong> <strong>College</strong> maintains the following records as required under<br />

the OSHA Laboratory Standard and other relevant OSHA standards. These records will<br />

be made available to employees, to the administration, or to any outside inspection<br />

agencies upon request.<br />

1. An official <strong>Chemical</strong> <strong>Hygiene</strong> <strong>Plan</strong>:<br />

This plan is located at the Health & Safety Office at A204. It is available for<br />

review between 9:00 AM and 3:00 PM from Monday through Friday. It is also<br />

available for review online at<br />

2. Laboratory employee training records<br />

All attendance sheets for laboratory employee training programs will be<br />

maintained by the EHSO and each department within <strong>Queensborough</strong><br />

<strong>Community</strong> <strong>College</strong> will also maintain a list of all laboratory employees and<br />

attendance sheets for that particular department.<br />

3. Incident reports/illnesses and injury logs<br />

The <strong>Chemical</strong> <strong>Hygiene</strong> Officer will maintain records of all laboratory incidents<br />

filed that occur in <strong>Queensborough</strong> <strong>Community</strong> <strong>College</strong> and any follow up action<br />

taken in response to the incident. Records of injuries and illnesses that have been<br />

documented by the <strong>Chemical</strong> <strong>Hygiene</strong> Officer will be maintained and posted<br />

regularly as required. These records will be kept at the office of Health & Safety<br />

A204.<br />
