This proposal should be attached separately. Additionally, Appendix D & F are provided for your convenience and contain annual usage and spend breakdown by category. Please be sure to quote the specific manufacturer listed, for Core Items and include the percent of discount off list for the Non-Core categories listed. In the event a comparable item of the same quality is available at a reduced price, please fill in the Vendor Item (If Different) field and provide details of the suggested substitute. Please quote both a central delivery model as well as a desktop delivery model and include a scenario that includes and excludes toner. If a central delivery model is chosen a monthly rebate is expected. Scenario 2 Appendix B contains the suggested Core List and Non-Core categories for UCHC located in Farmington, Connecticut. Additionally, Appendix E & F are provided for your convenience and contain annual usage and spend breakdown by category. Please be sure to quote the specific manufacturer listed, for Core Items and include the percent of discount off list for the Non-Core categories listed. In the event a comparable item of the same quality is available at a reduced price, please fill in the Vendor Item (If Different) field and provide details of the suggested substitute. Please quote assuming a central delivery model. Please refer to section 4.0 for specific requirements that apply to UCHC. Please note UCHC has different exclusions than Storrs. Scenario 3 Appendix C contains the suggested Core List and Non-Core categories for Storrs and Regional Campuses as well as UCHC located in Farmington, Connecticut. Additionally, Appendix D, E, & F are provided for your convenience and contain annual usage and spend breakdown by category. Please be sure to quote the specific manufacturer listed, for Core Items and include the percent of discount off list for the Non-Core categories listed. In the event a comparable item of the same quality is available at a reduced price, please fill in the Vendor Item (If Different) field and provide details of the suggested substitute. Please quote both a central delivery model as well as a desktop delivery model for Storrs only and central delivery for 18
UCHC. Please also include a scenario that includes and excludes toner for both. If a central delivery model is chosen a monthly rebate is expected. 3.7.1 Prices quoted shall remain firm fixed for a twenty-four month period. Thereafter, prices may be subject to revision, at yearly intervals. Price changes will be accepted subject to the following conditions only: 3.7.2 The proposal should include suggested procedures for dialog and agreement regarding the review of requested price increases in other similar mutual circumstances. After the initial period, the prices quoted in response to this Request for Proposal may be adjusted up or down in an amount not to exceed the Consumer Price Index (CPI), appropriate for the commodity, as published by the United States Department of Labor. 3.7.3 Price changes shall be submitted in writing thirty (30) calendar days prior to the date the increase rate is allowed to become effective, acceptance of which shall be subject to the University’s receipt and approval of the written documentation requesting the changes. No retroactive increases will be allowed. Price escalation is permitted only once per term. 3.7.4 Price changes approved by the University shall then remain firm for the term of each successive extension. 3.7.5 The prices quoted for each yearly period shall be firm except that the University shall receive any reduction in prices that may occur during the contract period. The reduced prices shall apply on all shipments made on or after the date the reduced prices become effective. 3.7.6 As the needs of the campus change regarding frequently purchased items and supplies, the University expects the successful bidder will monitor and apply the recurrent items to the core list so that the core list will change to reflect our buying patterns. 3.8 Market Price Adjustments, Audits, and Best Price Guarantee a.) By submitting a proposal, the respondent understands and agrees that the University will audit net prices for reasonableness of pricing. To determine reasonableness, contract prices will be compared with current market prices available from competitors. Price reviews will be performed periodically throughout the contract period. b.) If during the course of the contract and/or as a result of an audit, the prices of contract items are discovered to be unreasonably higher than current market prices, the University reserves the right to require and receive price adjustments from the Contractor. All adjustments shall be made to bring prices in line with competitive pricing for such items in the marketplace. c.) A best price guarantee will be required.
